CONSORTIUM PARTNERS INCLUDE THE MINNESOTA ELDER JUSTICE COALITION, OHIO COALITION FOR ADULT PROTECTIVE SERVICES, THE NATIONAL NETWORK OF STATE ELDER JUSTICE COALITIONS, AND JACQUE GRAY, PHD, SPECIALIST IN NATIVE AMERICAN ELDER JUSTICE AFFAIRS. THE NATIONAL ELDER JUSTICE COALITION CENTER, WILL SUPPORT THE DEVELOPMENT OF NEW STATE AND TRIBAL COALITIONS, WILL OFFER ONGOING TRAINING AND TECHNICAL ASSISTANCE TO NEW AND EXISTING COALITIONS, AND WILL SERVE AS A CLEARINGHOUSE FOR RESOURCES AND INFORMATION CONCERNING ELDER JUSTICE ISSUES, PROGRAMS, AND ADVOCACY. IN THE FIRST YEAR OF OPERATION THE CENTER WILL ISSUE A COMPETITIVE REQUEST FOR PROPOSALS FOR TWO-YEAR FUNDING FOR SEVEN GRANTS FOR THE DEVELOPMENT OF NEW STATEWIDE OR TRIBAL COALITIONS OR FOR THE ENHANCEMENT OF EXISTING STATE AND TRIBAL COALITIONS. THE PRIMARY FOCUS AREA OF THIS PROJECT IS HEALTHY FUTURES. THE REMAINING 50 RSVP VOLUNTEERS WILL SERVE TO SUPPORT OTHER COMMUNITY PRIORITIES. SOME OF THEIR ACTIVITIES WILL INCLUDE SERVICE AS STATE CERTIFIED OMBUDSMAN, TO ADVOCATE FOR INDIVIDUALS IN NURSING HOMES. OTHERS WILL SERVE AS COUNSELORS IN LIFESPANS FINANCIAL MANAGEMENT PROGRAM. THEY WILL PROVIDE IN HOME SUPPORT WITH BUDGETING, BALANCING CHECKBOOKS AND BILL PAYING. AT THE END OF THE THREE-YEAR GRANT, WE ANTICIPATE 250 PARTICIPANTS WILL HAVE COMPLETED MATTER OF BALANCE, LIVING HEALTHY WITH DIABETES AND/OR TAI CHI FOR ARTHRITIS WORKSHOPS WITH 95 % INDICATING THEY WILL MAKE CHANGES TO LIFESTYLE BASED UPON THE INFORMATION SHARED. VOLUNTEER DRIVERS WILL HAVE TRANSPORTED 350 UNDUPLICATED INDIVIDUALS TO MEDICAL APPOINTMENTS. THIS INTERVENTION WILL POSITIVELY IMPACT THE INDIVIDUAL'S PERCEPTION OF HAVING IMPROVED ACCESS TO MEDICAL CARE. GOAL IS 95% INDICATING IMPROVEMENT. RSVP NATIONAL VETERAN CORPS MEMBERS WILL HAVE SERVED 25 FELLOW VETERANS BY PROVIDING A SOURCE OF COMPANIONSHIP AND SOCIAL SUPPORT. AS A RESULT, WE PROJECT AN IMPROVEMENT IN PRE-VS POST SCORES AS MEASURED BY THE DUKE SOCIAL SUPPORT INDEX AND/ OR UCLA LONELINESS SCALE. VOLUNTEERS PLACED IN MEALS ON WHEELS ASSIGNMENTS WILL HAVE SERVED 500 MEALS TO HOMEBOUND OLDER ADULTS AND INDIVIDUALS WITH DISABILITIES, THUS INCREASING FOOD SECURITY. THE GOAL OF THE PROPOSED PROJECT IS TO REDUCE THE RISK OF REVICTIMIZATION AND TO SUPPORT MAXIMAL INDEPENDENT FUNCTIONING BY PROVIDING LONG TERM, COORDINATED SOCIAL, HEALTH-RELATED, AND FINANCIAL SERVICES. THIS CONSORTIUM AGREEMENT WILL ENABLE MEMBERS TO ENGAGE IN COLLABORATIVE MOBILITY MANAGEMENT PROJECTS AND TO ENTER INTO CONTRACTS ON BEHALF OF THE CONSORTIUM. LIVINGSTON COUNTY WILL EXTEND ITS CONTRACT WITH CAMBRIDGE SYSTEMATICS TO OTHER COUNTIES IN THE CONSORTIUM TO EXPAND THE USE OF THE 1-CLICK WEB PORTAL. IN TURN ENHANCEMENTS TO THE PLATFORM SPECIFIC TO SCHEDULING AND PURCHASING TRIPS WILL BE THE FOCUS OF THIS ICAM GRANT. EACH PARTNER COUNTY WILL DESIGNATE A LOCAL MOBILITY MANAGER TO COORDINATE THEIR COUNTY MOBILITY MANAGEMENT SERVICES WITH THE PILOTS REGIONAL MOBILITY MANAGER. LIFESPAN AND THE REGIONAL MOBILITY MANAGER WILL IMPLEMENT THE PILOT ON BEHALF OF THE CONSORTIUM.; EXPECTED OUTCOMES: THE SPECIFIC OUTCOMES OF THIS PROJECT ARE: 1. EXECUTIVE SUMMARY-AN ESTIMATED 200 RSVP VOLUNTEERS WILL SERVE. OF THIS NUMBER, 150 RSVP VOLUNTEERS WILL BE PLACED IN OUTCOME ASSIGNMENTS. SOME OF THEIR ACTIVITIES WILL INCLUDE COACHING EVIDENCE- BASED FALLS PREVENTION AND LIVING HEALTHY WORKSHOPS, DRIVING OLDER ADULTS AND INDIVIDUALS WITH DISABILITIES TO MEDICAL APPOINTMENTS, PROVIDING VETERAN TO VETERAN COMPANIONSHIP AND SUPPORT, AND DELIVERING NUTRITIOUS MEALS TO THE HOMES OF OLDER ADULTS AND INDIVIDUALS WITH DISABILITIES. THE PRIMARY FOCUS AREA OF THIS PROJECT IS HEALTHY FUTURES. THE REMAINING 50 RSVP VOLUNTEERS WILL SERVE TO SUPPORT OTHER COMMUNITY PRIORITIES. SOME OF THEIR ACTIVITIES WILL INCLUDE SERVICE AS STATE CERTIFIED OMBUDSMAN, TO ADVOCATE FOR INDIVIDUALS IN NURSING HOMES. OTHERS WILL SERVE AS COUNSELORS IN LIFESPANS FINANCIAL MANAGEMENT PROGRAM. THEY WILL PROVIDE IN HOME SUPPORT WITH BUDGETING, BALANCING CHECKBOOKS AND BILL PAYING. VOLUNTEERS SERVING OTHER COMMUNITY PRIORITIES WILL PROVIDE A SAFETY NET TO CATCH FINANCIAL AND OR HEALTHCARE ISSUES BEFORE THEY BECOME OUT OF CONTROL. THESE PREVENTATIVE MEASURES WILL HELP CREATE A BETTER QUALITY OF LIFE FOR ALL IN RECEIPT OF SERVICES. AT THE END OF THE THREE-YEAR GRANT, WE ANTICIPATE 250 PARTICIPANTS WILL HAVE COMPLETED MATTER OF BALANCE, LIVING HEALTHY WITH DIABETES AND/OR TAI CHI FOR ARTHRITIS WORKSHOPS WITH 95 % INDICATING THEY WILL MAKE CHANGES TO LIFESTYLE BASED UPON THE INFORMATION SHARED. VOLUNTEER DRIVERS WILL HAVE TRANSPORTED 350 UNDUPLICATED INDIVIDUALS TO MEDICAL APPOINTMENTS. THIS INTERVENTION WILL POSITIVELY IMPACT THE INDIVIDUAL'S PERCEPTION OF HAVING IMPROVED ACCESS TO MEDICAL CARE. GOAL IS 95% INDICATING IMPROVEMENT. RSVP NATIONAL VETERAN CORPS MEMBERS WILL HAVE SERVED 25 FELLOW VETERANS BY PROVIDING A SOURCE OF COMPANIONSHIP AND SOCIAL SUPPORT. AS A RESULT, WE PROJECT AN IMPROVEMENT IN PRE-VS POST SCORES AS MEASURED BY THE DUKE SOCIAL SUPPORT INDEX AND/ OR UCLA LONELINESS SCALE. VOLUNTEERS PLACED IN MEALS ON WHEELS ASSIGNMENTS WILL HAVE SERVED 500 MEALS TO HOMEBOUND OLDER ADULTS AND INDIVIDUALS WITH DISABILITIES, THUS INCREASING FOOD SECURITY. THE CNCS FEDERAL INVESTMENT OF $ 126,779 WILL BE SUPPLEMENTED BY $ 57,000.

ALL THREE PROGRAMS FOCUS ON ADDRESSING THE IMMINENT CRISIS OF MISTREATMENT AND EXPLOITATION UNTIL THE RISK OF FURTHER MISTREATMENT IS ELIMINATED OR MITIGATED. THE GOAL OF THE PROPOSED PROJECT IS TO REDUCE THE RISK OF REVICTIMIZATION AND TO SUPPORT MAXIMAL INDEPENDENT FUNCTIONING BY PROVIDING LONG TERM, COORDINATED SOCIAL, HEALTH-RELATED, AND FINANCIAL SERVICES. THE PROJECT WILL ACHIEVE THE FOLLOWING OBJECTIVES: 1) PROJECT STAFF WILL LINK CLIENTS TO LIFESPAN’S EXTENSIVE MENU OF SUPPORTIVE SERVICES OR TO OTHERS IN THE COMMUNITY AND FACILITATE ACCESS TO FINANCIAL MANAGEMENT AND BENEFITS OVER A PERIOD OF SIX TO EIGHTEEN MONTHS; 2) PROJECT STAFF WILL DEVELOP A PROTOCOL TO EVALUATE THE IMPACT OF THE PROGRAM ON CLIENT FUNCTIONING AND SAFETY USING EVIDENCE-INFORMED INSTRUMENTS IN CONCERT WITH PROJECT EVALUATOR, SUNY BROCKPORT. APPLICATION PURPOSE: THIS PILOT PROJECT WILL EXPAND THE CAPABILITIES OF THE FINGER LAKES REGIONAL 1-CLICK WEB PORTAL TO INCLUDE SCHEDULING AND PAYMENT OF TRIPS PROVIDED BY PUBLIC TRANSIT AND A NON-TRANSIT COMMUNITY TRANSPORTATION PROVIDER RESULTING IN AN INTEGRATED AND SEAMLESS PROCESS FOR ACCESSING COMMUNITY TRANSPORTATION.; ACTIVITIES PERFORMED: THE THREE MUNICIPAL PARTNER ARE DEVELOPING A REGIONAL MOBILITY MANAGEMENT CONSORTIUM AGREEMENT PURSUANT TO SECTION 119-0 OF THE NEW YORK STATE GENERAL MUNICIPAL LAW. THIS CONSORTIUM AGREEMENT WILL ENABLE MEMBERS TO ENGAGE IN COLLABORATIVE MOBILITY MANAGEMENT PROJECTS AND TO ENTER INTO CONTRACTS ON BEHALF OF THE CONSORTIUM. LIVINGSTON COUNTY WILL EXTEND ITS CONTRACT WITH CAMBRIDGE SYSTEMATICS TO OTHER COUNTIES IN THE CONSORTIUM TO EXPAND THE USE OF THE 1-CLICK WEB PORTAL. IN TURN ENHANCEMENTS TO THE PLATFORM SPECIFIC TO SCHEDULING AND PURCHASING TRIPS WILL BE THE FOCUS OF THIS ICAM GRANT. EACH PARTNER COUNTY WILL DESIGNATE A LOCAL MOBILITY MANAGER TO COORDINATE THEIR COUNTY MOBILITY MANAGEMENT SERVICES WITH THE PILOTS REGIONAL MOBILITY MANAGER. LIFESPAN AND THE REGIONAL MOBILITY MANAGER WILL IMPLEMENT THE PILOT ON BEHALF OF THE CONSORTIUM.; EXPECTED OUTCOMES: THE SPECIFIC OUTCOMES OF THIS PROJECT ARE: 1. THE HIRING OF A REGIONAL MOBILITY MANAGER TO OVERSEE THE DEVELOPMENT OF ENHANCEMENTS TO THE 1-CLICK MOBILITY PLATFORM. THIS POSITION WILL RESIDE AT LIFESPAN AND WILL COORDINATE WITH THE INDIVIDUAL COUNTIES AND THEIR MOBILITY MANAGERS THE REGIONAL TRANSIT SYSTEM AND CAMBRIDGE SYSTEMATICS. 2. THE GOAL OF THE ICAM PROJECT IS TO INCREASE ACCESS TO EXISTING TRANSPORTATION RESOURCES BY OLDER ADULTS PERSONS WITH DISABILITIES AND LOW-INCOME INDIVIDUALS RESIDING IN THE GENESEE-FINGER LAKES REGION OF NEW YORK. 3. THE IMPLEMENTATION OF PURCHASING RTS RIDE TICKETS THROUGH THE 1-CLICK PLATFORM. 4. THE COMPLETION OF A TECHNICAL DISCOVERY THAT ESTIMATES THE FEASIBILITY AND LEVEL OF EFFORT FOR ADDITIONAL TRIP BOOKING AND FARE PAYMENT INTEGRATION WITH A REGIONAL DEMAND RESPONSIVE TRANSPORTATION PROVIDER. 5.
