EXPECTED OUTCOMES INCLUDE THE STRENGTHENING OF A MULTI-DISCIPLINARY TEAM AND ITS DATA-INFORMED SERVICE DELIVERY, IMPROVED MENTAL HEALTH ACCESS AND OUTCOMES FOR DLIVE PARTICIPANTS, IMPROVEMENT IN SDOH INDICES OF DLIVE PARTICIPANTS AND THEIR FAMILIES, AND PREVENTION OF FUTURE INJURY AND DEATH.
