JOHN’S HOSPITAL, AND MULTIPLE HEALTHCARE FOR THE HOMELESS DELIVERY SITES. CONGRESSIONAL DISTRICTS WITHIN CCHC’S SERVICE AREA INCLUDE DISTRICT 13, AND DISTRICT 18. CCHC HAS BEEN IN OPERATION SINCE 1999. IN 2023, CCHC SAW 16,471 PATIENTS THROUGH 53,060 ENCOUNTERS ACROSS ALL ITS SITES AND SERVICES, INCLUDING JUST MORE THAN 1,619 VIRTUAL CARE VISITS CONSISTING MOSTLY OF BEHAVIORAL HEALTH CARE. CCHC IS REQUESTING $600,000 IN YEAR 1 AND $500,000 IN YEAR 2 (FOR A TOTAL OF $1,100,000 DURING THE PROJECT PERIOD) TO DEVELOP AND IMPLEMENT A TELEPSYCHIATRY PROGRAM AND EXPAND CURRENT BEHAVIORAL HEALTH SERVICES INCLUDING INCREASING THE NUMBER OF PATIENTS WITH ACCESS TO SUBSTANCE USE DISORDER TREATMENT. CCHC PROPOSES TO UTILIZE FUNDS TO IMPLEMENT A VARIETY OF INNOVATIVE MEANS OF ADDRESSING THESE ISSUES INCLUDING THE FOLLOWING: GROW ITS CURRENT CLINICAL STAFF SUPPORTING THE BEHAVIORAL HEALTH PROGRAM WHICH WILL INCREASE ACCESS TO SERVICES FOR ALL; EXPAND BEHAVIORAL HEALTH SUPPORT STAFF TO BETTER COORDINATE SERVICES SUCH AS SUD SERVICES AND TELEPSYCHIATRY; IMPLEMENT TELEPSYCHIATRY SERVICES FOR PATIENTS AS THERE IS VERY LIMITED ACCESS TO THIS SERVICE IN THE AREA AND VIRTUALLY NO ACCESS TO PATIENTS WITHOUT INSURANCE OR WITH MEDICAID.
