| Department of Health and Human Services | ESTROGEN-RELATED RECEPTOR AGONISTS FOR THE TREATMENT OF HEART FAILURE. - PROJECT SUMMARY/ABSTRACT. THIS SBIR PROPOSES DEVELOPMENT OF A NEW DRUG FOR TREATMENT OF HEART FAILURE, A DISEASE CURRENTLY RESPONSIBLE FOR THE DEATHS OF ~80,000 PEOPLE ANNUALLY IN THE US. THE UNDERLYING CAUSES OF THIS CONDITION INCLUDE MYOCARDIAL INFARCTION, HYPERTENSION AND DISEASE OF THE HEART VALVES. EXISTING TREATMENTS PRIMARILY ACT ON THE NEUROHORMONAL AXIS TO REDUCE VOLUME OVERLOAD BUT DESPITE THE AVAILABILITY OF THESE DRUGS, 5-YEAR MORTALITY FOLLOWING A HEART FAILURE DIAGNOSIS IS APPROXIMATELY 50% AND IS SIMILAR TO MANY CANCERS. WE PROPOSE TO DEVELOP A DRUG THAT DIRECTLY TARGETS CARDIAC METABOLISM, REVERSING THE REDUCED USE OF FAT OXIDATION THAT OCCURS IN THE FAILING HEART AND THEREBY ENABLING IT TO MAINTAIN ITS PUMP FUNCTION. THIS WILL BE ACHIEVED BY DRUGS THAT ACTIVATE ESTROGEN-RELATED RECEPTORS (ERRS), A TYPE OF NUCLEAR RECEPTOR THAT REGULATES MITOCHONDRIAL ENERGY METABOLISM. WE HAVE GENERATED DATA TO SHOW THAT ERR AGONISTS THAT ACTIVATE ALL 3 ISOFORMS PROTECT AGAINST THE DECLINE IN HEART FUNCTION THAT OCCURS IN THE TRANSAORTIC CONSTRICTION (TAC) MODEL IN THE MOUSE. IN THIS PHASE I APPLICATION WE PROPOSE TO IDENTIFY AGONISTS THAT SELECTIVELY ACTIVATE INDIVIDUAL ISOFORMS (OR COMBINATIONS THEREOF) AND TEST THEIR EFFECTS IN THE TAC MODEL IN ORDER TO DELINEATE THE ISOFORMS MEDIATING BENEFIT IN HEART FAILURE. WE HAVE SELECTED A SHORT-LIST OF POTENTIALLY USEFUL TOOLS FROM THE ~560 PROPRIETARY COMPOUNDS THAT HAVE ALREADY BEEN MADE AND WILL PROFILE THEM IN VITRO AND IN VIVO TO IDENTIFY THOSE SUITABLE TO BE TESTED IN THE TAC MODEL. IN THE COURSE OF THIS WORK, WE WILL ALSO VALIDATE ISOFORM-SPECIFIC GENETIC BIOMARKERS FOR THEIR ABILITY TO CONFIRM THE ACTIVATION OF THE SPECIFIC ISOFORMS AFTER IN VIVO DOSING OF COMPOUNDS. THIS PHASE 1 SBIR PROJECT PROPOSES DEVELOPMENT OF A NEW TYPE OF DRUG TO IMPROVE THE TREATMENT OF CANCER. THIS NEW APPROACH OFFERS POTENTIAL IN MANY FORMS OF THE DISEASE, INCLUDING BREAST CANCER, WHICH IS SECOND ONLY TO CANCER OF THE LUNG IN TERMS OF MORTALITY. IN PARTICULAR, TRIPLE NEGATIVE BREAST CANCER (TNBC) IS VERY CHALLENGING TO TREAT AS THERE ARE FEWER THERAPEUTIC OPTIONS COMPARED TO OTHER FORMS OF BREAST CANCER. RECENT ADDITIONS TO THE ONCOLOGY ARMAMENTARIUM INCLUDE IMMUNOTHERAPIES, SUCH AS IMMUNE CHECKPOINT INHIBITORS (ICBI) TARGETING PD-1, PD-L1 OR CTLA4, WHICH OFFER GREAT PROMISE BUT STILL ONLY BENEFIT A MINORITY OF TNBC PATIENTS. ACETYL COA SYNTHETASE 2 (ACSS2) IS A NUCLEOCYTOPLASMIC ENZYME THAT IS INVOLVED IN THE CONVERSION OF ACETATE TO ACETYL COA FOR USE AS A SUBSTRATE FOR FATTY ACID BIOSYNTHESIS AND FOR ACETYLATION OF NUCLEAR PROTEINS. ACSS2 IS COMMONLY UPREGULATED IN TUMOR CELLS AND HIGH EXPRESSION PREDICTS POOR OUTCOMES IN MULTIPLE CANCER TYPES INCLUDING TNBC. TUMOR CELLS USE ACETATE DERIVED FROM THE GUT MICROBIOME AS A SUBSTRATE TO SUPPORT THEIR ONGOING PROLIFERATION AND METABOLIC NEEDS. IN CONTRAST, THE RELATED ENZYMES ACSS1 AND ACSS3 ARE BOTH MITOCHONDRIALLY LOCATED AND THEREBY REGULATE A DIFFERENT POOL OF ACETYL COA FROM ACSS2 AND ARE RARELY UPREGULATED IN CANCER. CONSISTENT WITH ITS ASSOCIATION WITH HUMAN DISEASE OUTCOMES, GENETIC DELETION OF ACSS2 IMPAIRS TUMOR GROWTH IN TUMOR MODELS IN ANIMALS AND SMALL MOLECULE INHIBITORS OF ACSS2 PROVIDE BENEFICIAL EFFECTS IN MOUSE MODELS, INCLUDING TNBC MODELS THAT ARE RESISTANT TO ICBI. THE ACSS2 KNOCKOUT MOUSE HAS A NORMAL PHENOTYPE SUGGESTING THAT INHIBITION OF THIS ENZYME IS LIKELY TO HAVE A GOOD SAFETY/TOLERABILITY PROFILE. THIS SBIR PROJECT WILL EXPLORE THE POTENTIAL OF ACSS2 INHIBITORS ALONE AND IN COMBINATION WITH OTHER THERAPEUTIC MECHANISMS IN MODELS OF TNBC AND WILL FURTHER THE DEVELOPMENT OF A PROPRIETARY SERIES OF ACSS2 INHIBITORS TOWARDS THE AIM OF AN ORALLY AVAILABLE DRUG CANDIDATE SUITABLE FOR CLINICAL DEVELOPMENT. ESTROGEN-RELATED RECEPTOR AGONISTS FOR THE TREATMENT OF HEART FAILURE. - PROJECT SUMMARY/ABSTRACT. THIS SBIR PROPOSES DEVELOPMENT OF A NEW DRUG FOR TREATMENT OF HEART FAILURE, A DISEASE CURRENTLY RESPONSIBLE FOR THE DEATHS OF ~80,000 PEOPLE ANNUALLY IN THE US. THE UNDERLYING CAUSES OF THIS CONDITION INCLUDE MYOCARDIAL INFARCTION, HYPERTENSION AND DISEASE OF THE HEART VALVES. EXISTING TREATMENTS PRIMARILY ACT ON THE NEUROHORMONAL AXIS TO REDUCE VOLUME OVERLOAD BUT DESPITE THE AVAILABILITY OF THESE DRUGS, 5-YEAR MORTALITY FOLLOWING A HEART FAILURE DIAGNOSIS IS APPROXIMATELY 50% AND IS SIMILAR TO MANY CANCERS. WE PROPOSE TO DEVELOP A DRUG THAT DIRECTLY TARGETS CARDIAC METABOLISM, REVERSING THE REDUCED USE OF FAT OXIDATION THAT OCCURS IN THE FAILING HEART AND THEREBY ENABLING IT TO MAINTAIN ITS PUMP FUNCTION. THIS WILL BE ACHIEVED BY DRUGS THAT ACTIVATE ESTROGEN-RELATED RECEPTORS (ERRS), A TYPE OF NUCLEAR RECEPTOR THAT REGULATES MITOCHONDRIAL ENERGY METABOLISM. THERE ARE 3 ISOFORMS OF ESTROGEN-RELATED RECEPTORS – A, B AND G. WE HAVE GENERATED DATA TO SHOW THAT ERR AGONISTS THAT ACTIVATE ALL 3 ISOFORMS PROTECT AGAINST THE DECLINE IN HEART FUNCTION THAT OCCURS IN THE TRANSAORTIC CONSTRICTION (TAC) MODEL IN THE MOUSE. IN THIS PHASE I APPLICATION WE PROPOSE TO IDENTIFY AGONISTS THAT SELECTIVELY ACTIVATE INDIVIDUAL ISOFORMS (OR COMBINATIONS THEREOF) AND TEST THEIR EFFECTS IN THE TAC MODEL IN ORDER TO DELINEATE THE ISOFORMS MEDIATING BENEFIT IN HEART FAILURE. WE HAVE SELECTED A SHORT-LIST OF POTENTIALLY USEFUL TOOLS FROM THE ~560 PROPRIETARY COMPOUNDS THAT HAVE ALREADY BEEN MADE AND WILL PROFILE THEM IN VITRO AND IN VIVO TO IDENTIFY THOSE SUITABLE TO BE TESTED IN THE TAC MODEL. IN THE COURSE OF THIS WORK, WE WILL ALSO VALIDATE ISOFORM-SPECIFIC GENETIC BIOMARKERS FOR THEIR ABILITY TO CONFIRM THE ACTIVATION OF THE SPECIFIC ISOFORMS AFTER IN VIVO DOSING OF COMPOUNDS. IN PHASE II WE WILL USE THE KNOWLEDGE GAINED IN PHASE I TO DEVELOP MORE POTENT AND DRUG-LIKE ERR AGONISTS, NOMINATE A CLINICAL DEVELOPMENT CANDIDATE AND CHARACTERIZE ITS PROPERTIES MORE FULLY, INCLUDING IN HEART FAILURE MODELS OTHER THAN THE TAC MODEL. DEVELOPMENT OF A NOVEL APPROACH TO IMMUNOTHERAPY. - PROJECT SUMMARY/ABSTRACT. THIS PHASE 1 SBIR PROJECT PROPOSES DEVELOPMENT OF A NEW TYPE OF DRUG TO IMPROVE THE TREATMENT OF CANCER. THIS NEW APPROACH OFFERS POTENTIAL IN MANY FORMS OF THE DISEASE, INCLUDING BREAST CANCER, WHICH IS SECOND ONLY TO CANCER OF THE LUNG IN TERMS OF MORTALITY. IN PARTICULAR, TRIPLE NEGATIVE BREAST CANCER (TNBC) IS VERY CHALLENGING TO TREAT AS THERE ARE FEWER THERAPEUTIC OPTIONS COMPARED TO OTHER FORMS OF BREAST CANCER. RECENT ADDITIONS TO THE ONCOLOGY ARMAMENTARIUM INCLUDE IMMUNOTHERAPIES, SUCH AS IMMUNE CHECKPOINT INHIBITORS (ICBI) TARGETING PD-1, PD-L1 OR CTLA4, WHICH OFFER GREAT PROMISE BUT STILL ONLY BENEFIT A MINORITY OF TNBC PATIENTS. ACETYL COA SYNTHETASE 2 (ACSS2) IS A NUCLEOCYTOPLASMIC ENZYME THAT IS INVOLVED IN THE CONVERSION OF ACETATE TO ACETYL COA FOR USE AS A SUBSTRATE FOR FATTY ACID BIOSYNTHESIS AND FOR ACETYLATION OF NUCLEAR PROTEINS. ACSS2 IS COMMONLY UPREGULATED IN TUMOR CELLS AND HIGH EXPRESSION PREDICTS POOR OUTCOMES IN MULTIPLE CANCER TYPES INCLUDING TNBC. TUMOR CELLS USE ACETATE DERIVED FROM THE GUT MICROBIOME AS A SUBSTRATE TO SUPPORT THEIR ONGOING PROLIFERATION AND METABOLIC NEEDS. IN CONTRAST, THE RELATED ENZYMES ACSS1 AND ACSS3 ARE BOTH MITOCHONDRIALLY LOCATED AND THEREBY REGULATE A DIFFERENT POOL OF ACETYL COA FROM ACSS2 AND ARE RARELY UPREGULATED IN CANCER. CONSISTENT WITH ITS ASSOCIATION WITH HUMAN DISEASE OUTCOMES, GENETIC DELETION OF ACSS2 IMPAIRS TUMOR GROWTH IN TUMOR MODELS IN ANIMALS AND SMALL MOLECULE INHIBITORS OF ACSS2 PROVIDE BENEFICIAL EFFECTS IN MOUSE MODELS, INCLUDING TNBC MODELS THAT ARE RESISTANT TO ICBI. THE ACSS2 KNOCKOUT MOUSE HAS A NORMAL PHENOTYPE SUGGESTING THAT INHIBITION OF THIS ENZYME IS LIKELY TO HAVE A GOOD SAFETY/TOLERABILITY PROFILE. THIS SBIR PROJECT WILL EXPLORE THE POTENTIAL OF ACSS2 INHIBITORS ALONE AND IN COMBINATION WITH OTHER THERAPEUTIC MECHANISMS IN MODELS OF TNBC AND WILL FURTHER THE DEVELOPMENT OF A PROPRIETARY SERIES OF ACSS2 INHIBITORS TOWARDS THE AIM OF AN ORALLY AVAILABLE DRUG CANDIDATE SUITABLE FOR CLINICAL DEVELOPMENT. IF SUCCESSFUL WE HOPE TO SIGNIFICANTLY IMPROVE THE THERAPEUTIC OPTIONS FOR TNBC PATIENTS, IMPACTING DIRECTLY ON IMPROVED SURVIVAL AND QUALITY OF LIFE. INDIVIDUALS WITH DISABILITIES ARE HISTORICALLY UNEMPLOYED AND THOSE THAT ARE WORKING AREOFTEN UNDEREMPLOYED. THROUGH THIS TRAINING, OUR GOAL IS TO PROVIDE THE OPPORTUNITY FOR GRADUATESTO OBTAIN MEANINGFUL EMPLOYMENT IN THE LUCRATIVE AND GROWING IT FIELD THROUGH MODIFIEDINSTRUCTION, DELIVERY AND CAREER DEVELOPMENT SERVICES.ACTIVITIES TO BE PERFORMED:TEACH MARKETABLE INFORMATION TECHNOLOGY (IT) SKILLS, ALONG WITH APPROPRIATE WORK BEHAVIORSOVER 14 CONSECUTIVE WEEKS OF REMOTE INSTRUCTION (MONDAY THROUGH FRIDAY) FOLLOWING COMPTIASIT FUNDAMENTALS AND A CURRICULUM DELIVERED BY AN INSTRUCTOR FROM COMPTIA. AN ADDITIONAL HALFHOUR FOR EXTRA HELP IS PROVIDED ONE HALF HOUR PRIOR TO THE REGULAR CLASS. THE MODE AND STRUCTUREOF THE TRAINING WILL BE ADAPTED TO MEET THE NEEDS AND LEARNING STYLES OF QUALIFIED INDIVIDUALS WITHDISABILITIES. THE DELIVERY IS EXECUTED BY WAY OF INSTRUCTOR-LED TRAINING VIA ZOOM, HOMEWORK ANDFOLLOW UP EXERCISES, HANDS-ON PRACTICE WITH COMPUTER HARDWARE AND SOFTWARE, WORK READINESSSKILLS TRAINING AND JOB MATCHING SERVICES (THROUGH THE TCS VOCATIONAL REHABILITATION UNIT ANDCOMPTIA INSTRUCTOR) INCLUDING MOCK INTERVIEWS, JOB SEARCHING TECHNIQUES AND RESUMEDEVELOPMENT.EXPECTED OUTCOMES: AFTER THE 14 WEEKS OF INSTRUCTION, COMPLETION OF THE CURRICULUM ENTAILS THEPASSING OF AN EXAMINATION FOR EACH CORE AREA. WITH THE CERTIFICATION AND CAREER SERVICES TRAININGPROVIDED THROUGH THE PROGRAM, STUDENTS WILL OBTAIN AN ENTRY LEVEL POSITION IN THE IT FIELD.INTENDED BENEFICIARIES: ADULTS 18 WITH A DISABILITY IN NEW YORK, WITH THE POTENTIAL TO EXPANDOUTSIDE OF NEW YORK AS THE PROGRAM GROWS.
