CAN OPERATES THREE SITES AND PROPOSES TO OPEN A FOURTH SITE 2.6 MILES FROM CAN'S CLOSEST EXISTING SERVICE SITE. SINCE ITS FOUNDING IN 2015 AS A 501(C)(3) CORPORATION, CAN HAS ESTABLISHED ITSELF AS A COLLABORATIVE, HIGH-QUALITY PROVIDER OF COMPREHENSIVE MEDICAL, BEHAVIORAL HEALTH, AND SUPPORT SERVICES, FOCUSING ON REDUCING HEALTH INEQUITIES IN OUR COMMUNITY. CAN RECEIVED ITS FQHC-LAL DESIGNATION IN JUNE 2017. THE SERVICE AREA IS COMPRISED OF 122,247 INDIVIDUALS LIVING IN ZIP CODES 24501, 24502, 24503, AND 24504 (LYNCHBURG, VA); 24572 (AMHERST COUNTY, VA); AND 24588 (CAMPBELL COUNTY, VA). CAN'S TARGET POPULATION OF 18,812 RESIDENTS ACCOUNTS FOR 15% OF THE SERVICE AREA'S POPULATION. CAN'S COMMITMENT TO COMPREHENSIVE CARE IS EVIDENT IN THE FULL RANGE OF SERVICES IT PROVIDES. THESE INCLUDE GENERAL PRIMARY MEDICAL CARE, DIAGNOSTIC LABORATORY, DIAGNOSTIC RADIOLOGY, HEALTH SCREENINGS, VOLUNTARY FAMILY PLANNING, IMMUNIZATIONS, WELL CHILD SERVICES, GYNECOLOGICAL CARE, OBSTETRICAL CARE, PREVENTIVE DENTAL, PHARMACEUTICAL SERVICES, CASE MANAGEMENT, ELIGIBILITY ASSISTANCE, HEALTH EDUCATION, OUTREACH, TRANSPORTATION, TRANSLATION AND COVERAGE FOR EMERGENCIES DURING AND AFTER HOURS. ADDITIONALLY, CAN PROVIDES INTEGRATED MENTAL HEALTH AND SUBSTANCE ABUSE SERVICES, PSYCHIATRY, AND INFECTIOUS DISEASE SERVICES, INCLUDING RYAN WHITE PARTS B AND C. CAN CURRENTLY OPERATES AT THREE PERMANENT SITES: THE 5TH STREET COMMUNITY HEALTH CENTER, PARK VIEW COMMUNITY MISSION, AND THE CAN DENTAL CENTER. CAN HAS A ROBUST REDUCED FEE PROGRAM, WITH A NOMINAL FEE OF $0 FOR PATIENTS WITH INCOMES BELOW 100% FPL, INCREASING TO $5 FOR A PRIMARY CARE VISIT FOR PATIENTS BETWEEN 151% AND 200% FPL. CAN WILL USE NAP FUNDING TO HIRE ONE MIDLEVEL PROVIDER TO INCREASE CAPACITY BY 600 UNIQUE PATIENTS. THE LOCATION OF THE NEW NAP SITE IS WITHIN THE JUBILEE FAMILY DEVELOPMENT CENTER FACILITY AND IS NEXT TO A SECTION 8 VOUCHER DEVELOPMENT, EXPANDING OUTREACH TO THIS COMMUNITY. JUBILEE FAMILY DEVELOPMENT CENTER FOCUSES ON YOUTH AND FAMILIES. CAN PROPOSES TO SERVE 9,900 NEW PATIENTS WITH APPROXIMATELY 35,000 PATIENT ENCOUNTERS ACROSS FOUR DELIVERY SITES IN 2026. THE ADDRESSES FOR THE THREE EXISTING SITES AND NEW SITE ARE: 800 5TH STREET, MARTIN LUTHER KING, JR. BLVD, SUITE A, LYNCHBURG, VA, 24504 2420 MEMORIAL AVENUE, LYNCHBURG, VA, 24501 3718 OLD FOREST ROAD, LYNCHBURG, VA, 24501 1512 FLORIDA AVENUE, LYNCHBURG, VA, 24501
