QUALITY IMPROVEMENT FUND - CRESCENT COMMUNITY HEALTH CENTER (CRESCENT) WILL IMPLEMENT A TWO-YEAR QUALITY IMPROVEMENT PROJECT TO INCREASE ACCESS TO PREVENTIVE AND ADDITIONAL DENTAL SERVICES FOR CHILDREN AND YOUTH UNDER AGE 21 WITH NEURODEVELOPMENTAL DISORDERS (NDDS). CHILDREN AND YOUTH UNDER AGE 21 WITH NDDS IN CRESCENT’S SERVICE AREA FACE INCREASED ORAL HEALTH RISKS AND SIGNIFICANT BARRIERS TO PREVENTIVE AND ADDITIONAL DENTAL SERVICES. THESE BARRIERS INCLUDE ALMOST NO AVAILABILITY OF MEDICAID-ACCEPTING DENTAL PROVIDERS AND/OR PROVIDERS PREPARED TO SERVE CHILDREN WITH NDDS, SENSORY AND COMMUNICATION NEEDS, ANXIETY, MOBILITY OR BEHAVIORAL CHALLENGES, DIFFICULTY COMPLETING ROUTINE ORAL HEALTH BEHAVIORS, TRANSPORTATION AND SCHEDULING BARRIERS, AND THE NEED FOR ADDITIONAL CAREGIVER SUPPORT AND CARE COORDINATION. BUILDING ON IDENTIFIED PEDIATRIC PATIENTS WITH NDDS, THE PROJECT WILL IDENTIFY NEW PATIENTS WITH NDDS FROM THROUGHOUT THE SERVICE AREA, USE DATA TO PRIORITIZE OUTREACH, STRENGTHEN SCHEDULING AND REFERRAL WORKFLOWS, PROVIDE PATIENT AND CAREGIVER SUPPORT, TRAIN DENTAL HEALTH CENTER STAFF, IMPROVE/EXPAND A CARE COORDINATION MODEL, GATHER CAREGIVER FEEDBACK, AND MONITOR OUTCOMES THROUGH CONTINUOUS QUALITY IMPROVEMENT. CRESCENT WILL USE QIF-DNDD FUNDS FOR ALLOWABLE, SCOPE-ALIGNED ACTIVITIES THAT SUPPORT THE PROJECT PURPOSE, WHICH INCLUDE OFFERING DENTAL SERVICES CUSTOMIZED TO PEDIATRIC NDD PATIENTS AT THE MAIN CLINIC BUILDING AND THROUGH A NEW MOBILE DENTAL UNIT ALREADY IN SCOPE THAT WILL ADVANCE AND INNOVATE COMMUNITY OUTREACH. STAFFING, TRAINING, EQUIPMENT, FACILITY, AND PATIENT SUPPORT ACTIVITIES WILL BE ALIGNED WITH THE BUDGET NARRATIVE.
