THE TWO COUNTIES ARE HIGHLY RURAL, MODERATELY DIVERSE IN TERMS OF RACE AND ETHNICITY, AND RESIDENTS FACE NUMEROUS SOCIAL DETERMINANTS OF HEALTH (SDOH). UWD AND SCHC HAVE ENGAGED A WIDE RANGE OF COMMUNITY PARTNERS INCLUDING THE STATE DEPARTMENT OF PUBLIC HEALTH, PAYERS, CURRENTLY PRACTICING CHWS, HEALTH AND SOCIAL SERVICE PROVIDERS, COMMUNITY-BASED ORGANIZATIONS, COMMUNICATIONS AND MARKETING PROFESSIONALS, PAYERS, AND PATIENT ADVOCATES TO COLLABORATE TO ACHIEVE THE FOLLOWING GOALS AND OBJECTIVES: GOAL 1: ESTABLISH A CHW CERTIFICATION BOARD TO OVERSEE CHW CERTIFICATION AND EXPANSION IN THE TARGET AREA THROUGH 9/15/2025 OBJECTIVE 1.1: PROVIDE INPUT AND STRATEGIC DIRECTION FOR THE DEVELOPMENT AND ADMINISTRATION OF A CHW CERTIFICATION PROGRAM THAT IS VOLUNTARY AND PROVIDES APPROVED TRAINING TO MEET ESTABLISHED COMPETENCIES TO FULFILL THE ROLE AND RESPONSIBILITIES OF THE PROFESSIONAL CHW. OBJECTIVE 1.2 PROVIDE INPUT AND STRATEGIC DIRECTION FOR THE CURRICULUM DEVELOPMENT COMMITTEE TO DEVELOP A CURRICULUM FOR THE CHW TRAINING PROGRAM BY 2/14/23. OBJECTIVE 1.3: PROVIDE INPUT AND STRATEGIC DIRECTION FOR FOUR (4) REGISTERED APPRENTICESHIP AND FIELD PLACEMENT SITES – THREE (3) IN SUSSEX AND TWO (2) IN KENT- BY 2/14/23 GOAL 2: INCREASE THE NUMBER OF CERTIFIED CHWS IN SUSSEX AND KENT COUNTIES DELAWARE FROM 5 TO 200 BY 9/14/25 . OBJECTIVE 2.1: BY 3/14/23 DESIGN AND IMPLEMENT A COMPREHENSIVE DRWP COMMUNICATIONS EFFORT IN SUSSEX AND KENT COUNTIES TO RAISE COMMUNITY AWARENESS OF THE VIABILITY OF A CHW CAREER AND THE DRWP. OBJECTIVE 2.2: BY 9/14/2025 RECRUIT AND TRAIN 75 SUSSEX RESIDENTS AND 75 KENT RESIDENTS, WITH NO PRIOR EXPERIENCE AS A CHW OR HEALTH SUPPORT WORKER, AND 25 CURRENTLY PRACTICING CHWS IN SUSSEX AND 25 CURRENTLY PRACTICING CHWS IN KENT TO BECOME CERTIFIED CHWS. OBJECTIVE 2.3: ASSIGN 150 NEW CHW TRAINEES TO CHW FIELD PLACEMENTS AND REGISTERED APPRENTICESHIPS; AND 50 UPSKILLED CHW TRAINEES TO CHW FIELD PLACEMENTS.
