MMC WAS CHOSEN AS THE SITE FOR THIS PROJECT BECAUSE OF THE LARGE UNMET NEED THAT PERSISTS WITHIN MONMOUTH COUNTY, THE 5TH MOST POPULATED COUNTY OF NJ’S 21 COUNTIES. AS A SAFETY NET HOSPITAL, MMC’S PAYOR MIX IS HEAVILY WEIGHTED TOWARD CHARITY CARE AND SELF-PAY (12%) BUT WITHIN BEHAVIORAL HEALTH, THE PATIENT POPULATION IS EVEN LOWER-INCOME WITH A MEDICAID PATIENT POPULATION OF OVER 30%. THE MEDICALLY INDIGENT POPULATION IN OUR HOMETOWN OF LONG BRANCH AND THE SURROUNDING COMMUNITIES USE LOW-INCOME CLINICS PROVIDED THROUGH MMC AND THE ADJACENT FQHC AS THEIR ONLY SOURCE OF HEALTH CARE. OVER THE 24-MONTH PROJECT PERIOD, THIS NEW CCBHC WILL ALLOW MMC TO FOCUS ON KEY POPULATIONS AS FOLLOWS: 1) ADULTS WITH SERIOUS MENTAL ILLNESS; 2) ADULTS WITH SUBSTANCE ABUSE DISORDERS (SUD); 3) CHILDREN POST-CRISIS AND INPATIENT HOSPITALIZATION WITH SED; 4) PATIENTS WITH UNMET SUD AND MENTAL HEALTH NEEDS IN PRIMARY CARE SETTINGS. THE CCBHC WILL PRIORITIZE THE US VETERAN POPULATION AND HIGH RISK POPULATIONS IDENTIFIED IN RISK STRATIFICATION ASSESSMENTS. THE CCBHC WILL FOCUS ON TREATMENT ENGAGEMENT, REDUCING CRISES, AND PROVIDING THE HIGHEST QUALITY OF CARE TO THOSE WITH MULTI-MORBIDITIES TO ACHIEVE WHOLE-PERSON IMPROVEMENT OUTCOMES. THIS NEW APPROACH INVOLVES PHYSICIAN DIRECTED AND PROVIDED CARE AT THE SCENE OF EMERGENCIES OUT IN THE COMMUNITY IN ORDER TO ENHANCE HEALTH OUTCOMES AND SAVE LIVES. UNDER THIS INITIATIVE, RWJBH WILL EXPAND AND ENHANCE ITS EXISTING EMS PHYSICIAN TRAINING AND MOBILE HEALTH INITIATIVE CURRENTLY BASED IN NEWARK TO INCLUDE AN ADDITIONAL TEAM BASED AT COOPERMAN BARNABAS MEDICAL CENTER IN LIVINGSTON. BEYOND THE US MILITARY’S EXPERIENCE, RECENT STUDIES HAVE SHOWN THAT PRE-CLINICAL CARE CARRIED OUT BY EMS PHYSICIANS ASSURES BETTER RESULTS. MANY THERAPIES THAT ARE DELIVERED IN THE HOSPITAL CAN BE DEPLOYED IN THE FIELD, WHICH IS ONE OF THE MAJOR THRUSTS OF THE MILITARY “LESSONS LEARNED”. EMS PHYSICIANS ARE UNIQUELY POSITIONED TO INNOVATE THE DELIVERY OF THESE THERAPIES IN THE FIELD BECAUSE THEY ARE THE ONLY PROVIDER LEGALLY AUTHORIZED TO DELIVER THIS TYPE OF CARE IN THE CIVILIAN ENVIRONMENT. THE PHYSICIAN, IN THEIR RESPONSE VEHICLE, DELIVERS HOSPITAL LEVEL CARE DIRECTLY TO THE PATIENT. EVEN PRIOR TO ARRIVAL AT THE SCENE, THE PHYSICIAN STARTS DIRECTING CARE BY COMMUNICATING WITH ON-SCENE POLICE/FIRE PERSONNEL. THE MAJOR BENEFIT OF THE PHYSICIAN IS TO DELIVER IMMEDIATE LIFE-SAVING INTERVENTIONS WHILE ALSO ENHANCING TRIAGE CAPABILITIES, PROVIDING THE RIGHT RESOURCES TO THE RIGHT PATIENT AT THE RIGHT TIME.
