LIFESPAN OF GREATER ROCHESTER (LIFESPAN) WILL SERVE AS THE LEAD AGENCY OF A CONSORTIUM OF ELDER JUSTICE ORGANIZATIONS AND SPECIALISTS WITH MANY YEARS OF EXPERIENCE AND DEPTH OF EXPERTISE IN THE FIELD TO ESTABLISH A NATIONAL ELDER JUSTICE COALITION CENTER (NEJCC). CONSORTIUM PARTNERS INCLUDE THE MINNESOTA ELDER JUSTICE COALITION, OHIO COALITION FOR ADULT PROTECTIVE SERVICES, THE NATIONAL NETWORK OF STATE ELDER JUSTICE COALITIONS, AND JACQUE GRAY, PHD, SPECIALIST IN NATIVE AMERICAN ELDER JUSTICE AFFAIRS. THE EXPECTED OUTCOMES OF THE RRDPHWP ARE TO INCREASE AWARENESS AND DEVELOP BEST PRACTICES/TOOLS FOR RECRUITING AND RETAINING A DIVERSE PUBLIC HEALTH WORKFORCE FOR PUBLIC HEALTH STAKEHOLDERS. A WELL-TRAINED, RACIALLY AND ETHNICALLY DIVERSE PUBLIC HEALTH WORKFORCE CAN HAVE A CENTRAL ROLE IN MITIGATING HEALTH DISPARITIES AND IS A CRITICAL COMPONENT OF ANY EQUITABLE HEALTH CARE SYSTEM. THE RRDPHWP WILL COORDINATE: 1) AN ADVISORY COMMITTEE OF A DOZEN PUBLIC HEALTH, NATIONAL ASSOCIATION AND ACADEMIC DIVERSITY EXPERTS; 2) A LITERATURE SEARCH FOR RECRUITMENT AND RETENTION AND PROGRESSION ACTIVITIES TO INCREASE DIVERSITY IN PUBLIC HEALTH; 3) THREE FACILITATED REGIONAL SUMMITS AND INTERVIEWS/FOCUS GROUPS TO OBTAIN FEEDBACK FROM KEY THOUGHT LEADERS, PUBLIC HEALTH AND ACADEMIC EXPERTS ON BEST PRACTICES AND BARRIERS TO RECRUITMENT AND RETENTION OF A RACIALLY AND ETHNICALLY DIVERSE PUBLIC HEALTH WORKFORCE; 4) DEVELOP A RECRUITMENT AND RETENTION AND PROGRESSIVE STRATEGY WITH AI IN COLLABORATION WITH MINORITY INSTITUTIONS, PUBLIC HEALTH ASSOCIATIONS AND THE CDC; 5) COORDINATE A HEALTH COMMUNICATIONS CAMPAIGN TO FIND DIVERSE PUBLIC HEALTH SCHOOL ALUMNI AND OTHERS INTERESTED IN PUBLIC HEALTH POSITIONS AT THE NATIONAL, STATE AND LOCAL LEVELS; 6) DEVELOP A TRACKING AND MONITORING SYSTEM FOR THE PROGRAM; AND 7) PRODUCE REPORTS BASED ON THE PROGRAM EVALUATION PLAN WITH CDC. THE RRDPHWP WILL USE MULTIPLE STRATEGIES TO IMPROVE THE RECRUITMENT AND RETENTION/PROGRESSION OF CLUSTERS OF RACIAL/ETHNIC CANDIDATES TO PURSUE CAREERS IN THE PUBLIC HEALTH SECTOR, EMPHASIZING UNDERREPRESENTED POPULATIONS FOR USE BY CDC AND LOCAL AND STATE PUBLIC HEALTH DEPARTMENTS. AS THE LARGEST AND FASTEST-GROWING MINORITY IN THE US, COUPLED WITH DISPROPORTIONATE RATES OF HIV/AIDS, HISPANICS ARE CRUCIALLY IMPORTANT TO PRIORITIZE IN HIV PREVENTION AND CARE EFFORTS, ESPECIALLY AMONG MSM AND TRANSGENDER WOMEN. THE NATIONAL HISPANIC MEDICAL ASSOCIATION (NHMA) PROPOSAL FOR PARTNERING AND COMMUNICATING TOGETHER (CATEGORIES A AND B) WILL INCREASE HIV AWARENESS, REDUCE NEW HIV INFECTIONS, IMPROVE HEALTH OUTCOMES AND SUPPORT CDC?S LET?S STOP HIV TOGETHER (TOGETHER) CAMPAIGN.THE PURPOSE OF THE NHMA PROJECT IS TO MAXIMIZE THE EFFECTIVENESS AND REACH OF THE TOGETHER CAMPAIGN BY STRATEGICALLY DISSEMINATING HIV PREVENTION DIGITAL AND SOCIAL MEDIA MESSAGING AND COMMUNICATIONS TO THE HISPANIC COMMUNITY, AS WELL AS IMPLEMENTING EVENTS IN KEY U.S. JURISDICTIONS THAT FOCUS ON HIV AWARENESS AND PREVENTION. NHMA WILL HOLD 10 EVENTS PER YEAR AT THE NHMA ANNUAL CONFERENCE AND CHAPTER FORUMS IN SOUTHERN CALIFORNIA, NEW YORK CITY, PUERTO RICO, AND KEY AREAS WITH PARTNERS TO REDUCE NEW HIV INFECTIONS AMONG HISPANICS AT HIGHEST RISK, INCREASE REFERRALS TO HEALTHCARE PROVIDERS AND CREATE IMPROVED HEALTH OUTCOMES FOR HISPANICS WITH HIV. SHORT-TERM OUTCOMES INCLUDE INCREASE EXPOSURE TO THE CDC?S HIV CAMPAIGN MATERIALS AND MESSAGES AND HIV-RELATED INFORMATION-SEEKING BEHAVIORS AMONG HISPANICS DISPROPORTIONATELY AFFECTED BY HIV, WITH HIV AND HEALTHCARE PROVIDERS; AND INCREASE PARTNER ENGAGEMENT. THESE WILL LEAD TO INTERMEDIATE OUTCOMES THAT INCLUDE INCREASE CROSS-COMMUNITY SUPPORT AND FAVORABLE ATTITUDES FOR AND INCREASE HIV PREVENTION, TESTING, AND REFERRAL STRATEGIES; INCREASE HIV-RELATED COMMUNICATION AMONG PARTNERS, INCREASE REFERRALS AND LINKAGE TO CARE AMONG HISPANICS WITH HIV, AND INCREASE ADOPTION OF HIV SCREENING, PREVENTI ON, AND CARE AND TREATMENT RECOMMENDATIONS BY HEALTH CARE PROVIDERS. THE PROJECT WILL LEVERAGE NHMA?S EXISTING NETWORKS, ORGANIZATIONAL STRUCTURES, AND COMMUNICATION PLATFORMS USING EVIDENCE-BASED APPROACHES THAT ACKNOWLEDGE THE DIVERSITY IN THE US HISPANIC COMMUNITY, INCLUDING DIFFERENCES IN MEDIA USE AND TRUST OF HEALTH INFORMATION SOURCES. A MASS MEDIA STRATEGY, INCLUDING AD PLACEMENTS AND BROADCASTS, WILL BE SUPPLEMENTED BY ONLINE DISSEMINATION ACTIVITIES, SOCIAL MEDIA STRATEGY, EMAIL COMMUNICATIONS, TAILORED NHMA AND PARTNER WEBSITES, UPDATED HISPANIC HEALTH PORTAL, AND A VIRTUAL HISPANIC HIV COMMUNITY OF PRACTICE. THESE STRATEGIES WILL BE COMPLEMENTED BY FACE-TO-FACE COMMUNICATION AND DISSEMINATION AT NHMA?S ANNUAL CONFERENCE (INCLUDING EDUCATIONAL SESSIONS AND NETWORKING ACTIVITIES), LOCAL CHAPTER FORUMS, WEBINARS OF MESSAGING OF THE TOGETHER CAMPAIGN. HF WILL PROVIDE FINANCIALASSISTANCE TO FRONTLINE WORKERS WHO WERE HIRED TO PROVIDE FARMING AND OR MEATPACKING WORK AT ANY POINTDURING THE COVID 19 PANDEMIC PERIOD BEGINNING JANUARY 2020. HISPANIC FEDERATION WILL SERVE AS THE LEADAPPLICANT AND HAS ESTABLISHED SUBAWARD PARTNERSHIPS WITH 39 TRUSTED CBOS FROM 19 STATES, PUERTO RICOAND THE U.S. VIRGIN ISLANDS. HF AND ITS PARTNERING CBOS WILL CONDUCT AN ESTIMATED 5,600 OUTREACH ACTIVITIESTO INFORM ELIGIBLE BENEFICIARIES OF THE AVAILABILITY OF THE FINANCIAL ASSISTANCE. OUTREACH ACTIVITIES WILL BECONDUCTED IN ENGLISH OR SPANISH DEPENDING ON THE POPULATION. THE RECIPIENT WILL FOCUS ON BENEFICIARY EDUCATION, COMMUNITY PARTNERSHIP, AND MASS MEDIA CAMPAIGNS OF RESIDENTIAL SOLAR PV AND BATTERY STORAGE DEPLOYMENTS. ALTHOUGH THE RECIPIENT WILL NOT BE RESPONSIBLE FOR CONSUMER PROTECTION SERVICES, THEY WILL LEVERAGE THEIR EDUCATION AND OUTREACH ROLE TO INTEGRATE BASIC EDUCATION AND ORIENTATION TO BENEFICIARIES ON CONSUMER PROTECTION RIGHTS AND REFERRALS TO AVAILABLE PROTECTION SERVICES. THIS AGREEMENT WILL BE EXECUTED IN TWO PHASES. PHASE 1 IS TO DEVELOP A DETAILED PROJECT PLAN. FOLLOWING A SUCCESSFUL GO/NO-GO DECISION, THE RECIPIENT WILL ENTER THE IMPLEMENTATION PHASE 2, AND BEGIN EXECUTING THE PROJECT PLAN. THE RECIPIENT WILL BE EVALUATED EACH TWELVE MONTHS TO ENSURE GOALS OF THE PROJECT PLAN ARE BEING MET.

YET, ELDER JUSTICE COALITIONS EXIST IN LESS THAN HALF THE STATES AND IN FEW TRIBAL COMMUNITIES. LIFESPAN OF GREATER ROCHESTER (LIFESPAN) WILL SERVE AS THE LEAD AGENCY OF A CONSORTIUM OF ELDER JUSTICE ORGANIZATIONS AND SPECIALISTS WITH MANY YEARS OF EXPERIENCE AND DEPTH OF EXPERTISE IN THE FIELD TO ESTABLISH A NATIONAL ELDER JUSTICE COALITION CENTER (NEJCC). CONSORTIUM PARTNERS INCLUDE THE MINNESOTA ELDER JUSTICE COALITION, OHIO COALITION FOR ADULT PROTECTIVE SERVICES, THE NATIONAL NETWORK OF STATE ELDER JUSTICE COALITIONS, AND JACQUE GRAY, PHD, SPECIALIST IN NATIVE AMERICAN ELDER JUSTICE AFFAIRS. FOR THE TARGET POPULATION OF PATIENTS CURRENTLY RECEIVING MAT PROVIDED BY WS IN THE AFOREMENTIONED COUNTIES: FOR PATIENTS WITH DOCUMENTED ETHNICITY, 77.6% NON-HISPANIC, 19.7% HISPANIC; RACE 74.5% WHITE, 7% BLACK/AFRICAN AMERICAN, .7% NATIVE AMERICAN, .3% ASIAN, .2% NATIVE HAWAIIAN OR OTHER PACIFIC ISLANDER, .2% MULTI-RACIAL; 64.6% OF THE PATIENTS ARE MALE, 35.4% OF THE PATIENTS ARE FEMALE WITH ONE PATIENT (.1%) IDENTIFIED AS TRANSGENDER; 6.8% OF THE PATIENTS ARE BETWEEN THE AGES OF 18-24, 42.5% 25-34, 30.7% 35-44, 12.4% 45-54, 5.1% 55-64 AND .2% 65+. THERE ARE OVER 20,000 INDIVIDUALS WITH OUD IN THE SERVICE AREA. THE TOTAL POPULATION SERVED BY WS IS 1,610,265 PATIENTS: 81% WHITE NON-HISPANIC, 11% HISPANIC OR LATINO, 5% BLACK NON-HISPANIC, AND 1% ASIAN; 2), AVERAGE PATIENT IS 44; 3) 50.7% OF PATIENTS ARE FEMALE, 49.3% OF PATIENTS ARE MALE, AND 94 PATIENTS HAVE DOCUMENTED IN THEIR ELECTRONIC HEALTH RECORD TRANS OR GENDER NON-CONFIRMING; 4) OVER 90% OF HOUSEHOLDS IN ADAMS, FRANKLIN, AND YORK COUNTIES SPEAK ONLY ENGLISH, 87% OF HOUSEHOLDS IN LEBANON COUNTY, AND 83% OF HOUSEHOLDS IN LANCASTER COUNTY SPEAK ONLY ENGLISH WITH THE MOST COMMON SECOND LANGUAGE BEING SPANISH; 5) AN AVERAGE OF 8.2% OF PERSONS LIVE IN POVERTY IN THE FIVE-COUNTY AREA, WITH THIS RATE INCREASING TO 28% (MORE THAN TWICE THE NATIONAL RATE) FOR YORK CITY WHERE WS SEES ITS HIGHEST POPULATION OF MAT PATIENTS. WSMAT PROJECT GOALS ARE TO INCREASE THE NUMBER OF NEW MAT INTAKES, INCREASE ENGAGEMENT, ABSTINENCE AND GPRA INTERVIEW COMPLETION AT 6 MONTHS, SUPPORT A HOLISTIC APPROACH TO SCREENING AND TREATMENT TO IMPROVE CLIENT OUTCOMES AND REDUCE COST OF CARE. OUTCOMES WILL BE 2,140 NEW MAT INTAKES INCLUDING SPANISH-SPEAKING PATIENTS, 3,000+ CALLS TO THE WARM LINE OVER 5 YEARS, AT LEAST 10 NEW PRESCRIBERS INCLUDING AT LEAST 1 IN RURAL FRANKLIN COUNTY, INCREASED ENGAGEMENT TO REACH 80% ENGAGEMENT AT 6 MONTHS, 90% REPORTING DECREASE IN ILLICIT OPIOID DRUG USE OR PRESCRIPTION OPIOID MISUSE AT 6 MONTHS POST-INTAKE, AND CONSISTENCY IN SCREENING FOR CO-OCCURRING DIAGNOSES, INFECTIOUS DISEASE, AND SOCIAL DETERMINANTS OF HEALTH.

TREATMENT OF INFLAMMATORY COMPLICATIONS OF RESPIRATORY INFECTION - ABSTRACT RESPANA THERAPEUTICS IS ADVANCING A PROPRIETARY FIRST-IN-CLASS THERAPEUTIC MONOCLONAL ANTIBODY (MAB), RT- 002, FOR THE TREATMENT OF DEBILITATING AND OFTEN LETHAL INFLAMMATORY COMPLICATIONS ASSOCIATED WITH ACUTE RESPIRATORY CONDITIONS. WE HAVE IDENTIFIED A LEAD CANDIDATE, RT-002, AND AN EQUALLY PROMISING ALTERNATIVE. RT-002 TARGETS THE SURFACTANT PROTEIN A RECEPTOR SP-R210, A HOST-MEDIATED TARGET THAT HOLDS THE PROMISE OF EFFICACY REGARDLESS OF PATHOGEN EVOLUTION. RIGOROUS IN VIVO MOUSE STUDIES HAVE DEMONSTRATED NOT ONLY REDUCTION IN MORTALITY BUT RESTORATION OF LUNG HEALTH AS WELL. FURTHERMORE, RT-002 DOES NOT TARGET AN INDIVIDUAL CYTOKINE PATHWAY, WHICH CAN HAVE NEGATIVE EFFECTS. RT-002 THUS HAS THE POTENTIAL TO FILL A SIGNIFICANT GAP IN THE THERAPEUTICS ARSENAL FOR INFLUENZA SINCE THE MOST WIDELY USED TOOLS ARE VACCINES TO PREVENT INFECTION AND ANTIVIRAL DRUGS TO STOP INFECTION; HOWEVER, THESE ARE LIMITED BY PARTIAL EFFICACY, VACCINE HESITANCY, AND EMERGENCE OF DRUG RESISTANCE. THE OBJECTIVE OF THIS PHASE II PROPOSAL IS TO CONTINUE DEVELOPMENT OF RT-002 AND THE BACKUP WITH THE GOAL OF ADVANCING THE THERAPEUTIC TOWARD CLINICAL TRIALS AS QUICKLY AND EFFICIENTLY AS POSSIBLE. SPECIFICALLY, THE AIMS INCLUDE: 1) DETERMINE RT-002 PHARMACOLOGICAL ACTION IN HUMAN BLOOD THROUGH OUR FUNCTIONAL IMMUNOPHENOTYPE ASSAYS TO DISCERN RT-002 TARGET ENGAGEMENT AND IMMUNE ACTIVATION IN HUMAN BLOOD THAT WILL INFORM THERAPEUTIC DESIGN IN ALLEVIATING ABERRANT IMMUNE ACTIVATION IN CRITICALLY ILL PATIENTS; 2) ESTABLISH PHARMACOLOGICAL ACTIVITY OF RT-002 TREATMENT FOR SEVERE INFLUENZA UTILIZING HUMANIZED FCRN MICE TO ESTABLISH SAFETY, TOXICITY, PHARMACODYNAMICS, DOSAGE, SCHEDULE, AND DURATION OF TREATMENT TO DE-RISK THE DEVELOPMENT OF RT-002 TOWARDS CLINICAL TRIALS IN HUMANS; AND 3) ESTABLISH FOUNDATIONS FOR PHASE I/IIA CLINICAL TRIALS FOR TRANSITION OF RT-002 TO CLINICAL TESTING THROUGH EXISTING PARTNERSHIPS WITH CONTRACTUAL RESEARCH ORGANIZATIONS, CRITICAL ILLNESS, AND CRITICAL TRIALS EXPERTS FOR PRODUCTION AND GMP CERTIFICATION OF RT-002 IN STABLE CHO CELLS, AND ORGANIZATION AND PLANNING OF CRITICAL PARAMETERS FROM AIMS 1 AND 2 THAT ARE NEEDED FOR PRE-IND CONSULTATION WITH THE FDA AND THE DESIGN OF PHASE I/IIA CLINICAL TRIALS. THE MULTI-SYSTEM ORGANIZATIONS SERVING THE TARGET POPULATION IN THESE TWO COUNTIES WILL COLLABORATE TO PROMOTE AWARENESS OF THE UNIQUE CHALLENGES FACED BY TRANSITIONAL-AGED YOUTH (TAY) WITH BEHAVIORAL HEALTH AND INTELLECTUAL-DEVELOPMENTAL DISABILITIES. OHTP WILL PROVIDE CULTURALLY COMPETENT SERVICES INCLUDING A FULL CONTINUUM OF LIFE-SKILLS, VOCATIONAL, EDUCATIONAL, AND SOCIAL-EMOTIONAL WELLNESS TO ENABLE YOUTH TO BECOME HEALTHY AND PRODUCTIVE ADULTS. SPECIFICALLY, THE FOLLOWING EVIDENCE-BASED PRACTICES WILL BE EXPANDED: TRANSITION TO INDEPENDENCE (TIP), TRAUMA-INFORMED COGNITIVE-BEHAVIOR THERAPY (TI-CBT), COGNITIVE BEHAVIORAL THERAPY FOR SUICIDE PREVENTION (CBT-SP) AND HIGH-FIDELITY WRAPAROUND. THE KEY FOCI OF THE OHTP WILL BE THE FOLLOWING: A) IMPROVE ACCESS TO SERVICES FOR TAY BY INCREASING YOUTH AND ADULT SYSTEM COORDINATION AS MEASURED BY PROCEDURE CHANGES, OHTP ADVISORY COUNCIL PERFORMANCE, STREAMLINED FUNDING, AND EHR SERVICE DATA, AND IMPROVING SERVICE CAPACITY AS MEASURED BY CASELOAD DATA AND PROCEDURE GUIDELINES; B) INCREASE REGIONAL AND STATE AWARENESS OF TAY NEEDS FOR SED/SMI AND IDD AS MEASURED BY SOCIAL MEDIA METRICS (NUMBER OF IMPRESSIONS, ETC.), COLLATERAL INFORMATION MATERIAL DISTRIBUTED, AND NUMBER OF NEW REFERRALS MENTIONING SOCIAL MARKETING AS SOURCE, AND C) INCREASE EMPLOYMENT, EDUCATION, TRAINING, AND COMMUNITY INVOLVEMENT FOR TARGET POPULATION AS MEASURED BY DISCHARGE RECORDS. THE EXPECTED OUTCOMES OF THE RRDPHWP ARE TO INCREASE AWARENESS AND DEVELOP BEST PRACTICES/TOOLS FOR RECRUITING AND RETAINING A DIVERSE PUBLIC HEALTH WORKFORCE FOR PUBLIC HEALTH STAKEHOLDERS. AS THE LARGEST AND FASTEST-GROWING MINORITY IN THE US, COUPLED WITH DISPROPORTIONATE RATES OF HIV/AIDS, HISPANICS ARE CRUCIALLY IMPORTANT TO PRIORITIZE IN HIV PREVENTION AND CARE EFFORTS, ESPECIALLY AMONG MSM AND TRANSGENDER WOMEN. THE NATIONAL HISPANIC MEDICAL ASSOCIATION (NHMA) PROPOSAL FOR PARTNERING AND COMMUNICATING TOGETHER (CATEGORIES A AND B) WILL INCREASE HIV AWARENESS, REDUCE NEW HIV INFECTIONS, IMPROVE HEALTH OUTCOMES AND SUPPORT CDC?S LET?S STOP HIV TOGETHER (TOGETHER) CAMPAIGN.THE PURPOSE OF THE NHMA PROJECT IS TO MAXIMIZE THE EFFECTIVENESS AND REACH OF THE TOGETHER CAMPAIGN BY STRATEGICALLY DISSEMINATING HIV PREVENTION DIGITAL AND SOCIAL MEDIA MESSAGING AND COMMUNICATIONS TO THE HISPANIC COMMUNITY, AS WELL AS IMPLEMENTING EVENTS IN KEY U.S. JURISDICTIONS THAT FOCUS ON HIV AWARENESS AND PREVENTION. NHMA WILL HOLD 10 EVENTS PER YEAR AT THE NHMA ANNUAL CONFERENCE AND CHAPTER FORUMS IN SOUTHERN CALIFORNIA, NEW YORK CITY, PUERTO RICO, AND KEY AREAS WITH PARTNERS TO REDUCE NEW HIV INFECTIONS AMONG HISPANICS AT HIGHEST RISK, INCREASE REFERRALS TO HEALTHCARE PROVIDERS AND CREATE IMPROVED HEALTH OUTCOMES FOR HISPANICS WITH HIV. SHORT-TERM OUTCOMES INCLUDE INCREASE EXPOSURE TO THE CDC?S HIV CAMPAIGN MATERIALS AND MESSAGES AND HIV-RELATED INFORMATION-SEEKING BEHAVIORS AMONG HISPANICS DISPROPORTIONATELY AFFECTED BY HIV, WITH HIV AND HEALTHCARE PROVIDERS; AND INCREASE PARTNER ENGAGEMENT. THESE WILL LEAD TO INTERMEDIATE OUTCOMES THAT INCLUDE INCREASE CROSS-COMMUNITY SUPPORT AND FAVORABLE ATTITUDES FOR AND INCREASE HIV PREVENTION, TESTING, AND REFERRAL STRATEGIES; INCREASE HIV-RELATED COMMUNICATION AMONG PARTNERS, INCREASE REFERRALS AND LINKAGE TO CARE AMONG HISPANICS WITH HIV, AND INCREASE ADOPTION OF HIV SCREENING, PREVENTI ON, AND CARE AND TREATMENT RECOMMENDATIONS BY HEALTH CARE PROVIDERS.
