WE ANTICIPATE IMPROVED POPULATION HEALTH OUTCOMES THROUGH IMPROVED COORDINATION OF BEHAVIORAL HEALTH AND PHYSICAL HEALTH CARE AND ENHANCED HEALTH INFORMATION TECHNOLOGY (HIT) SYSTEMS. LIFESKILLS CCBHC WILL DEVELOP A CARE COORDINATION PROGRAM TO INTEGRATE BETTER PHYSICAL HEALTH AND BEHAVIORAL HEALTH, A PRIMARY FOCUS FOR THE APPROXIMATELY 575 CONSUMERS WE WILL SERVE DURING THE FOUR YEARS OF THE PROJECT. WE WILL ENHANCE OUR HIT SYSTEMS IN ORDER TO BETTER COORDINATE, MONITOR AND TRACK OUTCOMES. THE CARE COORDINATION PROGRAM WILL BRIDGE THE GAP BETWEEN BEHAVIORAL AND PHYSICAL HEALTH CARE. LIFESKILLS CCBHC WILL PROVIDE A FULL SPECTRUM OF INTEGRATED SERVICES, INCLUDING TELEPHONIC, WALK-IN, AND MOBILE CRISIS SERVICES; COMPLETE AND DETAILED ASSESSMENT, OUTPATIENT PRIMARY CARE SCREENING/MONITORING OF KEY HEALTH INDICATORS; MEDICATION-ASSISTED TREATMENT; MEDICATION MANAGEMENT; AND TRAUMA-INFORMED, EVIDENCE-BASED OUTPATIENT MENTAL HEALTH AND SUBSTANCE USE SERVICES. WE WILL FOCUS ON SEVERAL EVIDENCE-BASED PRACTICES (EBP), INCLUDING COGNITIVE BEHAVIORAL THERAPY, MOTIVATIONAL INTERVIEWING, ASSERTIVE COMMUNITY TREATMENT, SUPPORTED EMPLOYMENT, AND SUPPORTED HOUSING. BUILDING ON THE PRINCIPLES OF WHOLE-PERSON CARE, OUR PERSON-CENTERED APPROACH, COMBINED WITH A RECOVERY-FOCUSED MODEL, WILL PLACE THE PERSON RECEIVING SERVICES AT THE CENTER OF ALL DECISION-MAKING. GOALS AND OBJECTIVES BEGIN WITH DECREASING BARRIERS TO SERVICES BY IMPLEMENTING IMPROVED SCHEDULING FOR PSYCHIATRIC CARE AND NEW APPOINTMENTS TO REDUCE THE WAIT TIME BETWEEN INITIAL EVALUATIONS AND FOLLOW-UP THERAPY APPOINTMENTS AND ENHANCING THE HIT SYSTEMS THAT WILL ADD EFFICIENCY FOR CONSUMERS SEEKING SERVICES. SECOND, WE WILL IMPROVE BEHAVIORAL HEALTH OUTCOMES BY HIRING A MINIMUM OF TWO CARE COORDINATORS TO PROVIDE CARE COORDINATION, IMPLEMENT BIOMETRIC SCREENINGS, METABOLIC AND BRIEF INTERVENTIONS, AND TRAIN CARE COORDINATORS ON THE CHRONIC CARE MODEL. THIS SMALL BUSINESS INNOVATION RESEARCH (SBIR) PHASE I PROJECT UTILIZES THE INTEGRATION OF AN LLM (LARGE LANGUAGE MODEL) AND RAG (RETRIEVAL AUGMENTED GENERATION) FRAMEWORK INTO A CAREER GAMING PLATFORM THAT CAREFULLY SELECTS AND PROVIDES CONTEXT FROM CONTROLLED SOURCES OF INFORMATION THAT THE AI USES TO GENERATE PERSONALIZED CAREER GUIDANCE. INCLUDED IN THIS INNOVATION IS THE DEVELOPMENT OF BENCHMARKS WHERE AN AI-JUDGE, FINE-TUNED ON THE RELEVANT DATA FROM A VECTOR DATABASE, IS LEVERAGED TO DETERMINE EFFICACY OF THE LLM TO UTILIZE KNOWLEDGE IN THE RAG PIPELINE. THE DEVELOPMENT OF BENCHMARKS AND ANALYTICS LAYERS AROUND THIS ENVIRONMENT CREATES A PERFORMANCE-DRIVEN CLOSED LOOP SYSTEM THAT ALLOWS FOR CONTINUOUS IMPROVEMENT. ADDITIONALLY, BY CONTROLLING THE DATASETS FROM WHICH THE AI RETRIEVES INFORMATION, THIS APPROACH MITIGATES THE RISK OF REPLICATING BIASES AND STEREOTYPES AND ENSURES DIVERSE PERSPECTIVES AND DATA SPECIFICALLY RELEVANT TO THE TARGET. THIS METHOD WILL ALSO REDUCE COMPUTATIONAL DEMANDS, ACCELERATE CONTENT UPDATES, AND IS SIGNIFICANTLY LESS COSTLY THAN FINE-TUNING A PRE-TRAINED LLM, ALLOWING FOR EASY ADAPTATIONS OF THE KNOWLEDGE IT HAS ACCESS TO. THIS PROGRAM EXPECTS TO SERVE OVER 1,000 ADULT IMMIGRANTS AND REFUGEES EACH YEAR FOR THREE YEARS. DURING THIS NEAR YEAR OF THE PROJECT, BUILDING SKILLS PARTNERSHIP IS PROPOSING TO COST-SHARE FIVE PLACEMENTS (5 COST-SHARE AND 2 REGULAR). THIS REQUEST IS FOR A 10TH AND FINAL YEAR CONTINUATION PROJECT THAT WILL BEGIN ON 09/03/2018 AND CONCLUDE ON 08/02/2020. BSP PROPOSES TO WORK IN MOUNTAIN VIEW, OAKLAND, SAN DIEGO, SANTA CLARA, ORANGE COUNTY, AND LOS ANGELES COUNTIES BY PROVIDING A MULTIFACETED APPROACH TO UPLIFTING FAMILIES FROM POVERTY. THIS APPLICATION PROPOSES TO CLOSE SPECIFIC PROGRAMMING, WHICH HAS BEEN SUPPORTED THROUGH THE CORPORATION FOR NATIONAL AND COMMUNITY SERVICE AND HAS NOW BEEN INSTITUTIONALIZED AND WILL CONTINUED TO BE SUSTAINED AT BSP. THESE VISTA'S WILL CONTINUE WORKING ON EXPANDING PROVEN EFFECTIVE PROGRAMMING TO RELATIVELY NEW REGIONS, WHICH INCLUDE SAN DIEGO, OAKLAND, AND MOUNTAIN VIEW. FOR THESE SITES, BSP IS REQUESTING THE FOLLOWING VISTA PLACEMENTS: 1 IN SAN DIEGO, 1 IN OAKLAND AND 2 IN MOUNTAIN VIEW (AT GOOGLE). OUR FOCUS POPULATIONS WILL BE CONSUMERS OF ALL AGE RANGES WHO ARE EXPERIENCING SERIOUS MENTAL ILLNESS (SMI), SEVERE EMOTIONAL DISTURBANCE (SED) AND/OR SUBSTANCE USE DISORDERS (SUD). WE ANTICIPATE IMPROVED POPULATION HEALTH OUTCOMES THROUGH IMPROVED COORDINATION OF BEHAVIORAL HEALTH AND PHYSICAL HEALTH CARE AND ENHANCED HEALTH INFORMATION TECHNOLOGY (HIT) SYSTEMS. LIFESKILLS CCBHC WILL DEVELOP A CARE COORDINATION PROGRAM TO INTEGRATE BETTER PHYSICAL HEALTH AND BEHAVIORAL HEALTH, A PRIMARY FOCUS FOR THE APPROXIMATELY 575 CONSUMERS WE WILL SERVE DURING THE FOUR YEARS OF THE PROJECT. WE WILL ENHANCE OUR HIT SYSTEMS IN ORDER TO BETTER COORDINATE, MONITOR AND TRACK OUTCOMES. THE CARE COORDINATION PROGRAM WILL BRIDGE THE GAP BETWEEN BEHAVIORAL AND PHYSICAL HEALTH CARE. LIFESKILLS CCBHC WILL PROVIDE A FULL SPECTRUM OF INTEGRATED SERVICES, INCLUDING TELEPHONIC, WALK-IN, AND MOBILE CRISIS SERVICES; COMPLETE AND DETAILED ASSESSMENT, OUTPATIENT PRIMARY CARE SCREENING/MONITORING OF KEY HEALTH INDICATORS; MEDICATION-ASSISTED TREATMENT; MEDICATION MANAGEMENT; AND TRAUMA-INFORMED, EVIDENCE-BASED OUTPATIENT MENTAL HEALTH AND SUBSTANCE USE SERVICES. WE WILL FOCUS ON SEVERAL EVIDENCE-BASED PRACTICES (EBP), INCLUDING COGNITIVE BEHAVIORAL THERAPY, MOTIVATIONAL INTERVIEWING, ASSERTIVE COMMUNITY TREATMENT, SUPPORTED EMPLOYMENT, AND SUPPORTED HOUSING. BUILDING ON THE PRINCIPLES OF WHOLE-PERSON CARE, OUR PERSON-CENTERED APPROACH, COMBINED WITH A RECOVERY-FOCUSED MODEL, WILL PLACE THE PERSON RECEIVING SERVICES AT THE CENTER OF ALL DECISION-MAKING. GOALS AND OBJECTIVES BEGIN WITH DECREASING BARRIERS TO SERVICES BY IMPLEMENTING IMPROVED SCHEDULING FOR PSYCHIATRIC CARE AND NEW APPOINTMENTS TO REDUCE THE WAIT TIME BETWEEN INITIAL EVALUATIONS AND FOLLOW-UP THERAPY APPOINTMENTS AND ENHANCING THE HIT SYSTEMS THAT WILL ADD EFFICIENCY FOR CONSUMERS SEEKING SERVICES. SECOND, WE WILL IMPROVE BEHAVIORAL HEALTH OUTCOMES BY HIRING A MINIMUM OF TWO CARE COORDINATORS TO PROVIDE CARE COORDINATION, IMPLEMENT BIOMETRIC SCREENINGS, METABOLIC AND BRIEF INTERVENTIONS, AND TRAIN CARE COORDINATORS ON THE CHRONIC CARE MODEL. THIS GEO-SPECIFICITY IS IMPERATIVE IN BREAKING CYCLES OF POVERTY AND KEEPING COMMUNITIES THRIVING. THE CORE OF THIS INNOVATION LIES IN A SYNERGISTIC COMBINATION OF KEY AI TECHNOLOGIES TO PROVIDE UNBIASED, PERSONALLY RELEVANT LEARNING EXPERIENCES WITH ACTIONABLE CAREER GUIDANCE AND MENTORSHIP THROUGH ENGAGING AND ADAPTIVE GAME MECHANICS. THE INSIGHTS GLEANED FROM THIS ENDEAVOR WILL EXTEND THE PROJECT?S IMPACT BEYOND ADVANCED MANUFACTURING CAREERS TO ADDITIONAL STEM INDUSTRIES IN AN EFFORT TO CLOSE SOCIAL EQUITY AND KNOWLEDGE GAPS AND FOSTER A MORE DIVERSE WORKFORCE AROUND THE WORLD. THIS SMALL BUSINESS INNOVATION RESEARCH (SBIR) PHASE I PROJECT UTILIZES THE INTEGRATION OF AN LLM (LARGE LANGUAGE MODEL) AND RAG (RETRIEVAL AUGMENTED GENERATION) FRAMEWORK INTO A CAREER GAMING PLATFORM THAT CAREFULLY SELECTS AND PROVIDES CONTEXT FROM CONTROLLED SOURCES OF INFORMATION THAT THE AI USES TO GENERATE PERSONALIZED CAREER GUIDANCE. INCLUDED IN THIS INNOVATION IS THE DEVELOPMENT OF BENCHMARKS WHERE AN AI-JUDGE, FINE-TUNED ON THE RELEVANT DATA FROM A VECTOR DATABASE, IS LEVERAGED TO DETERMINE EFFICACY OF THE LLM TO UTILIZE KNOWLEDGE IN THE RAG PIPELINE. THE DEVELOPMENT OF BENCHMARKS AND ANALYTICS LAYERS AROUND THIS ENVIRONMENT CREATES A PERFORMANCE-DRIVEN CLOSED LOOP SYSTEM THAT ALLOWS FOR CONTINUOUS IMPROVEMENT. ADDITIONALLY, BY CONTROLLING THE DATASETS FROM WHICH THE AI RETRIEVES INFORMATION, THIS APPROACH MITIGATES THE RISK OF REPLICATING BIASES AND STEREOTYPES AND ENSURES DIVERSE PERSPECTIVES AND DATA SPECIFICALLY RELEVANT TO THE TARGET. THIS METHOD WILL ALSO REDUCE COMPUTATIONAL DEMANDS, ACCELERATE CONTENT UPDATES, AND IS SIGNIFICANTLY LESS COSTLY THAN FINE-TUNING A PRE-TRAINED LLM, ALLOWING FOR EASY ADAPTATIONS OF THE KNOWLEDGE IT HAS ACCESS TO. THE COMBINATION OF THESE ADAPTIVE TECHNOLOGIES IS POISED TO SCALE EARLY CAREER DISCOVERY ACROSS ALL STEM INDUSTRIES, FOR ALL YOUTH, NO MATTER WHERE THEY LIVE OR WHAT THEY LOOK LIKE. THE SKILLS CENTER (TSC) IS SEEKING TO PROVIDE AN ARRAY OF PROVEN SPORTS-BASED YOUTH DEVELOPMENT SERVICES FOR MINORITY MIDDLE AND HIGH SCHOOL GIRLS AGES 11-17 WHO RESIDE OR ATTEND SCHOOL IN TARGETED ZIP CODES SURROUNDING TSCS NEW FACILITY. THESE ZIP CODES (33603, 33604, 33605, 33607, 33610, 33612, AND 33619) INCLUDE HIGHER PERCENTAGES OF MINORITIES, LOWER PERCENTAGES OF ADULTS WITH A HIGH SCHOOL DIPLOMA OR GED, AND HIGHER A PERCENTAGES OF INDIVIDUALS LIVING BELOW THE FEDERAL POVERTY LEVEL THAN EITHER TAMPA OR HILLSBOROUGH COUNTY. FAMILIES AND GIRLS IN THESE COMMUNITIES LACK GENDER-SPECIFIC ENRICHMENT PROGRAMS. TSCS APPROACH BUILDS STRATEGIES ON THE OJJDPS PRIORITIES RELATED TO YOUTH DEVELOPMENT ACTIVITIES THAT PREVENT AND/OR REDUCE YOUTH VIOLENCE AND EXPOSURE TO VIOLENCE. THROUGH TAMPA YES, TSC WILL PROVIDE SERVICES TO 50 ELIGIBLE GIRLS EACH OF THE THREE FUNDING YEARS. GIRLS WILL BE IDENTIFIED FROM THE TARGETED ZIP CODES BY TSC STAFF AND COMMUNITY. THE YOUTH WILL PARTICIPATE IN TRAUMA-INFORMED, POSITIVE YOUTH DEVELOPMENT PROGRAMMING THAT INCLUDES A INDIVIDUAL EDUCATION PLAN FOR EACH GIRL, TUTORING, MENTORING, RECREATIONAL PROGRAMMING, AND SERVICES FROM TSCS THREE OTHER MINORITY-LED COLLABORATORS. THE GIRLS PROGRESS WILL BE TRACKED THROUGH TSCS WEB-BASED DATA PLATFORM, SCOREBOARD. THESE METRICS ALONG WITH THOSE REQUIRED FOR OJJDP PERFORMANCE MANAGEMENT WILL VERIFY TAMPA YES SUCCESS IN PREVENTING YOUTH VIOLENCE AMONG GIRLS IN TAMPAS LOW-INCOME MINORITY COMMUNITIES. TAMPA YES PROMOTES RACIAL EQUITY AND REMOVES BARRIERS TO PARTICIPATION FOR LOW-INCOME, MINORITY GIRLS IN THE TARGETED ZIP CODES IN TAMPA. THE TAMPA YES PROGRAM IS ENHANCED BY TSCS IDENTIFICATION AS A CULTURALLY CONNECTED ORGANIZATION. FURTHERMORE, THE COLLABORATION WITH THREE OTHER BLACK-LED ORGANIZATIONS STRENGTHENS THE TAMPA YES SERVICE MODEL AND OPPORTUNITY FOR SUCCESS.

OUR PROPOSED VISTA PROJECT ALIGNS WITH THE CNCS FOCUS AREAS OF ECONOMIC OPPORTUNITY, THROUGH THE DEVELOPMENT OF A FINANCIAL LITERACY CURRICULUM, EDUCATION, THROUGH THE DEVELOPMENT OF A FAMILY LITERACY CURRICULUM, AND HEALTHY FUTURES, THROUGH THE DEVELOPMENT OF A HEALTH LITERACY CURRICULUM. THE VISTA PROJECT PROPOSED WILL PROVIDE HIGH QUALITY AND CULTURALLY APPROPRIATE FAMILY, FINANCIAL, AND HEALTH LITERACY CURRICULUM TO THE LOW-INCOME ADULTS IN OUR ESL CLASSES FOR UTAH REFUGEES AND IMMIGRANTS, STRENGTHENING OUR OVERALL PROGRAMS PROVIDING ESL AND CITIZENSHIP INSTRUCTION BY DEVELOPING NEW FUNDING RESOURCES TO SUPPORT THE ESLC FOR FUTURE IMPACT IN THE REFUGEE AND IMMIGRANT COMMUNITY. THIS PROGRAM EXPECTS TO SERVE OVER 1,000 ADULT IMMIGRANTS AND REFUGEES EACH YEAR FOR THREE YEARS. WE SEEK THREE FULL-TIME VISTA MEMBERS WHO WILL CONTRIBUTE TO THESE GOALS THROUGH THEIR WORK OF RESEARCHING, DEVELOPING, AND IMPLEMENTING ESL CURRICULUM WHICH SPECIFICALLY ADDRESSES THE TOPICS OF FAMILY LITERACY AND READING, FINANCIAL RESOURCE MANAGEMENT, AND LITERACY IN HEALTH CARE AND PREVENTION. THESE VISTA MEMBERS WILL ALSO CONTRIBUTE TO BUILDING THE CAPACITY OF THE ESLC FOR FUTURE SUSTAINABILITY AND IMPACT IN THE COMMUNITY OVER THE COURSE OF THE THREE YEAR PROJECT.

BUILDING SKILLS PARTNERSHIP (BSP) RESPECTFULLY REQUESTS THE SUPPORT FROM THE CORPORATION FOR NATIONAL AND COMMUNITY SERVICE FOR A 10TH AND FINAL YEAR OF SUPPORT TO CONTINUE TO DEEPEN, INCREASE PROGRAMMING, AND EXPORT OUR EFFECTIVE PROGRAMMING. THIS PROJECT WILL ASSIST IN BUILDING WITH THE EXPANSION OF PROGRAMMING TO A WIDER ARRAY OF WORKING FAMILIES IN THE SERVICE INDUSTRY, WHICH INCLUDE A HIGH CONCENTRATION OF AFRICAN-AMERICAN WORKERS AND THEIR FAMILIES. THE WORK OF VISTAS WILL BE IMPERATIVE AS BSP EXPANDS ITS INFRASTRUCTURE AND CAPACITY TO NOT ONLY REACH THESE POPULATIONS, BUT TO BE ABLE TO BUILD BRIDGES AND ALLIANCES BETWEEN ALL SERVICE WORKERS TO CREATE OPPORTUNITIES FOR UPWARD INCOME MOBILITY. DURING THIS NEAR YEAR OF THE PROJECT, BUILDING SKILLS PARTNERSHIP IS PROPOSING TO COST-SHARE FIVE PLACEMENTS (5 COST-SHARE AND 2 REGULAR). THIS REQUEST IS FOR A 10TH AND FINAL YEAR CONTINUATION PROJECT THAT WILL BEGIN ON 09/03/2018 AND CONCLUDE ON 08/02/2020. BSP PROPOSES TO WORK IN MOUNTAIN VIEW, OAKLAND, SAN DIEGO, SANTA CLARA, ORANGE COUNTY, AND LOS ANGELES COUNTIES BY PROVIDING A MULTIFACETED APPROACH TO UPLIFTING FAMILIES FROM POVERTY. THIS APPLICATION PROPOSES TO CLOSE SPECIFIC PROGRAMMING, WHICH HAS BEEN SUPPORTED THROUGH THE CORPORATION FOR NATIONAL AND COMMUNITY SERVICE AND HAS NOW BEEN INSTITUTIONALIZED AND WILL CONTINUED TO BE SUSTAINED AT BSP. FOR THIS APPLICATION, WE ARE CONTINUING TO PROPOSE TO NOT HOST A VISTA MEMBER IN OUR SAN JOSE OFFICE. BSP HAS ABSORBED PROGRAMMING AND SUSTAINED THIS OFFICE SINCE FY 2016-2017. IN ADDITION, BSP PROPOSES TO CONTINUE WITH OUR RESPONSIBILITY FOR COST-SHARE. FOR THIS PROJECT CONTINUATION, BSP PROPOSES TO CONTINUE OUR COST-SHARES TO 5 MEMBERS. COST-SHARE CITIES INCLUDE SAN DIEGO, ORANGE, AND LOS ANGELES. BSP IS REQUESTING THE SUPPORT FROM THE CORPORATION FOR NATIONAL AND COMMUNITY SERVICE FOR 7 VISTA'S 2 REGULAR AND 5 COST-SHARES. THESE VISTA'S WILL CONTINUE WORKING ON EXPANDING PROVEN EFFECTIVE PROGRAMMING TO RELATIVELY NEW REGIONS, WHICH INCLUDE SAN DIEGO, OAKLAND, AND MOUNTAIN VIEW. FOR THESE SITES, BSP IS REQUESTING THE FOLLOWING VISTA PLACEMENTS: 1 IN SAN DIEGO, 1 IN OAKLAND AND 2 IN MOUNTAIN VIEW (AT GOOGLE). FOR EXISTING SITES WHICH INCLUDE LOS ANGELES AND ORANGE COUNTY, BSP IS PROPOSING TO CONTINUE WITH IS STRATEGY TO BUILDING ITS COLLEGE ACCESS PROGRAM WHILE ALSO EXPANDING THE PROGRAM TO A WIDER ARRAY OF WORKING FAMILIES IN THE SERVICE INDUSTRY. ALL VISTAS WILL WORK TOWARDS DEVELOPING, STRENGTHENING, AND EXPANDING BSP'S COLLEGE ACCESS PROGRAMS, WHICH INCLUDE THE SUCCESSFUL TRANSITION FROM YOUTH TO ADULTHOOD. VISTAS WILL ASSIST IN DEVELOPING AN INFRASTRUCTURE THROUGH VOLUNTEERS, SYSTEMS, PARTNERSHIPS AND GRANTS TO SUPPORT OUR OBJECTIVE TO HELP STUDENT'S COMPLETE HIGH SCHOOL, ENROLL IN COLLEGE, AND ENTER IN THE WORKFORCE. WE STRONGLY BELIEVE THE ABOVE STRATEGY ALIGNS CLOSELY WITH THE CORPORATION FOR NATIONAL AND COMMUNITY SERVICE TO BREAK CYCLES OF POVERTY. IN ADDITION, BSP WILL PROVIDE TRAINING AND SERVICES TO HELP BOTH IMMIGRANT WORKERS AND AFRICAN-AMERICAN WORKERS TRANSITION FROM OUR VOCATIONAL PROGRAMS TO SUBSEQUENTLY CONTINUE TO HIGHER EDUCATION OR CAREER TRAINING, THUS INCREASING THEIR INCOME AND SOCIAL MOBILITY FOR THEMSELVES AND THEIR FAMILIES. SINCE RECEIVING ITS FIRST GRANT FROM THE CORPORATION FOR NATIONAL AND COMMUNITY SERVICE IN 2008, BSP HAS BEEN ABLE TO ACHIEVE IMPORTANT OPERATIONAL AND INFRASTRUCTURAL GOALS BECAUSE OF THE CONTRIBUTIONS MADE BY VISTA MEMBERS. BSP'S MISSION IS TO PROVIDE SERVICES, TRAINING AND EDUCATIONAL PROGRAM THAT IMPROVE THE QUALITY OF LIFE FOR LOW-WAGE PROPERTY SERVICE WORKERS AND THEIR FAMILIES BY INCREASING THEIR SKILLS, ACCESS TO EDUCATION, AND OPPORTUNITIES FOR CAREER AND COMMUNITY ADVANCEMENT. THE VISTA GRANT AND VISTA MEMBERS HAVE BEEN AN ESSENTIAL ELEMENT OF BSP'S SUCCESS.
