THE PROPOSED VISTA PROJECT ALIGNS WITH THE VISTA OPIOID PROGRAMMING FOCUS AREA. THE VISTA PROJECT WILL ENHANCE ASHF'S DATA ANALYTICS FOR THE PURPOSE OF IMPROVING TARGETED PROGRAM DELIVERY, WHILE SUPPORTING STAFF THROUGH TRAINING, CURRICULUM DEVELOPMENT, AND TARGETED OUTREACH. "THIS PROJECT WILL FOCUS ON PROVIDING EDUCATION AND TRAINING TO THE YOUTH/PARENTS OF JOHNSON COUNTY TO RAISE THE AWARENESS OF THE DANGERS OF UNDERAGE ALCOHOL USE AND DRINKING AND DRIVING." ACCORDING TO DATA USA STATISTICAL RECORDS JOHNSON COUNTY IS A COMMUNITY OF 17,923 FULL TIME RESIDENTS. THE AVERAGE HOUSEHOLD INCOME IS $30,180, A 26.9% POVERTY RATE WITH 100% OF OUR STUDENTS RECEIVING FREE LUNCH. WE'RE AN ISOLATED COMMUNITY, APPROXIMATELY 1 HOUR FROM THE NEAREST INTERSTATE AND THE NEAREST FULL SERVICE HOSPITAL. APPROXIMATELY 15% OF OUR RESIDENTS ARE UNINSURED AND MANY MORE UNDERINSURED. WE HAVE NO TREATMENT FACILITIES FOR ALCOHOL OR SUD'S WITHIN AN HOURS DRIVE. WE HAVE ONE AA MEETING FOR THE ENTIRE COUNTY WHICH COVERS APPROXIMATELY 300 SQUARE MILES. WE HAVE 1 SCHOOL SYSTEM WITH 5 ELEMENTARY SCHOOLS AND ONE MIDDLE SCHOOL AND ONE HIGH SCHOOL. APPROXIMATELY ¼ OF OUR COUNTY POPULATION IS UNDER THE AGE OF 24. WE HAVE LIMITED INDUSTRY IN OUR COMMUNITY AND VERY LIMITED EMPLOYMENT RESOURCES. JOHNSON COUNTY SCHOOL SYSTEM AND NORTHEAST CORRECTIONAL CENTER ARE THE LARGEST EMPLOYERS WITHIN THE COUNTY. OUR YOUTH HAVE LIMITED ACCESS TO RECREATION SUCH AS MOVIES, BOWLING, SHOPPING AND OTHER ACTIVITIES AS NONE OF THESE ARE LOCATED WITHIN OUR COUNTY SO THE COMMON PRACTICE IS TO HAVE DRINKING PARTIES IN HOMES AND ISOLATED AREAS IN OUR COMMUNITY. WE RECENTLY PASSED LEGISLATION TO GO FROM THE LAST REMAINING "DRY" COUNTY IN THE UPPER EAST TENNESSEE AREA TO A "WET" COUNTY. GOAL 1: DECREASE YOUTH ALCOHOL USE IN THE COMMUNITY BY 5% OVER THE NEXT 4 YEARS BY IMPLEMENTING EVIDENCE-BASED PROGRAMS WITHIN THE SCHOOL DISTRICT THAT ADDRESS BEHAVIORS THAT MAY LEAD TO THE INITIATION OF USE. GOAL 2: STRENGTHEN COLLABORATION ACROSS COMMUNITY SECTORS TO CHANGE SOCIAL PERCEPTION OF RISK OF HARM AMONG PARENTS AND COMMUNITY. THIS WILL REACH ALL 17,923 RESIDENTS OVER THE 4 YEAR PERIOD. OBJECTIVE 1: BY THE END OF THE SCHOOL YEAR 2023, WE WILL HAVE UTILIZED THE BOTVIN LIFESKILLS TRAINING WITH APPROXIMATELY 200 STUDENTS AGES 12-15 TO DECREASE ALCOHOL CONSUMPTION BY 5% AND INCREASE AWARENESS OF HARM BY 5% AT ALL ELEMENTARY SCHOOLS AND WILL HAVE TRAINED 3 SCHOOL PERSONNEL WITH THIS SYSTEM. SAMHSA FY2023 CONGRESSIONAL DIRECTIVE SPENDING PROJECTS - A SAFE HAVEN FOUNDATION (ASHF), ONE OF THE LARGEST TRANSITIONAL HOUSING PROVIDERS IN CHICAGO, PROPOSES TO CREATE AN ONSITE INTEGRATED BEHAVIORAL HEALTH CENTER AT ITS MAIN FACILITY IN ORDER TO INCREASE CAPACITY AND EXPAND SERVICES FOR PEOPLE EXPERIENCING HOMELESSNESS. THE INTEGRATED BEHAVIORAL HEALTH CENTER WILL INCLUDE CLINICAL TREATMENT FOR SUBSTANCE USE DISORDERS (SUDS), CO-OCCURRING DISORDERS (CODS), AND MENTAL HEALTH COUNSELING, AS WELL AS PRIMARY CARE AND PHARMACY SERVICES. THE GEOGRAPHIC CATCHMENT AREA IS ALL CHICAGO CONTINUUM OF CARE (IL-510) ENCOMPASSING A POPULATION OF FOCUS THAT SELF-IDENTIFIES AS: NON-HISPANIC BLACK (75%) OR HISPANIC/LATINX (11%); UNDER AGE 24 (24%) OR AGE 25 OR OLDER (76%); MALE (67%) OR FEMALE (32%); AND, AS HOMELESS HOUSEHOLDS WITH CHILDREN (25%). WITH THE SUPPORT OF ITS PARTNERS - RUSH SUBSTANCE USE DISORDER CENTER OF EXCELLENCE AT THE RUSH UNIVERSITY MEDICAL CENTER, WEST SIDE HEROIN/OPIOID TASK FORCE AND A SAFE HAVEN LLC. - THE ASHF INTEGRATED BEHAVIORAL HEALTH CENTER ANTICIPATES ASSESSING 5,000 INDIVIDUALS EXPERIENCING HOMELESSNESS OR AT RISK OF HOMELESSNESS, ANNUALLY, AND INTAKING APPROXIMATELY 1,000 (25%) FOR SUDS, CODS, AND/OR MENTAL HEALTH COUNSELING. DURING THE GRANT PERIOD OF PERFORMANCE, ASHF ENDEAVORS TO EARN CARF ACCREDITATION AND STATUS AS A CERTIFIED COMMUNITY BEHAVIORAL HEALTH CENTER WITH SUPPORT FROM MTM SERVICES LLC FOR ITS INTEGRATED BEHAVIORAL HEALTH CENTER. ASHF'S GOALS ARE 1) CREATE AN ONSITE INTEGRATED BEHAVIORAL HEALTH CENTER FOR CLIENTS IN ASHF'S TRANSITIONAL HOUSING SERVICES; 2) INCREASE OUTREACH ACTIVITIES TO INDIVIDUALS WHO ARE EXPERIENCING HOMELESSNESS AND WHO NEED OR FEEL THE NEED FOR SUDS/CODS TREATMENT OR MENTAL HEALTH COUNSELING; 3) REDUCE SUBSTANCE USE AMONG CLIENTS PARTICIPATING IN TREATMENT PROGRAMS FROM ASHF'S INTEGRATED BEHAVIORAL HEALTH CENTER; AND, 4) REDUCE SYMPTOMS AMONG CLIENTS DIAGNOSED WITH A MENTAL HEALTH DISORDER WHO RECEIVE TREATMENT FROM ASHF'S INTEGRATED BEHAVIORAL HEALTH CENTER. THE STRATEGIES AND INTERVENTIONS WILL INCLUDE A HOLISTIC APPROACH OF EVIDENCE-BASED PRACTICES FOR HOUSING FIRST, HARM REDUCTION, MOTIVATIONAL INTERVIEWING, INTENSIVE CASE MANAGEMENT, PEER SUPPORT AND COMMUNITY REINFORCEMENT. THE CENTER'S OBJECTIVES ON BEHALF OF PARTICIPANTS ARE A) TO TREAT 80% OF INDIVIDUALS EXPERIENCING HOMELESSNESS WHO ARE ASSESSED TO NEED SUDS/CODS/MENTAL HEALTH TREATMENT AND B) TO REDUCE THE USE OF SUBSTANCES AND MENTAL HEALTH SYMPTOMS FOR 80% OF CLIENTS WHO ENROLL IN TREATMENT PROGRAMS. BASED TYPICALLY ANNUAL VOLUME OF CLIENTS, ASHF INTEGRATED BEHAVIORAL HEALTH CENTER ANTICIPATES SCREENING 5,000 INDIVIDUALS PER YEAR, DIAGNOSING 1,000 CLIENTS (25%) AS CANDIDATES FOR SUDS/CODS/MENTAL HEALTH TREATMENT, INTAKING 800 CLIENTS (80% OF DIAGNOSES) FOR BEHAVIORAL HEALTH SERVICES, AND ASSISTING 640 CLIENTS (80% OF INTAKES) TO REDUCE USE OF SUBSTANCES, TO TO REDUCE SYMPTOMS OF CODS AND TRAUMA OVER THE COURSE OF THE YEAR.

THE KENT COUNTY SHERIFFS OFFICE AND SAFE HAVEN MINISTRIES PROPOSE TO IMPLEMENT THE WEST MICHIGAN HUMAN TRAFFICKING TASK FORCE. THE WEST MICHIGAN HUMAN TRAFFICKING TASK FORCES JURISDICTIONAL AREA IS PRIMARILY KENT COUNTY, MICHIGAN. THE AREA INCLUDES GRAND RAPIDS, THE SECOND-LARGEST CITY IN MICHIGAN WITH A POPULATION OF NEARLY 200,000 RESIDENTS AND A TOTAL COUNTY POPULATION JUST SHY OF 660,000 RESIDENTS. BY ENGAGING SURROUNDING COMMUNITIES, THE TASK FORCE WILL BUILD A COORDINATED RESPONSE THAT CAN EVENTUALLY EXPAND BEYOND COUNTY LINES TO ENSURE COUNTY BARRIERS DO NOT INTERFERE WITH THE PROTECTION OF AND JUSTICE FOR SURVIVORS. WITH PURPOSE AREA 1 FUNDING, THE WEST MICHIGAN HUMAN TRAFFICKING TASK FORCE WILL DEVELOP, EXPAND, AND STRENGTHEN THE LOCAL INFRASTRUCTURE FOR A MULTIDISCIPLINARY APPROACH TO HUMAN TRAFFICKING IDENTIFICATION AND RESPONSE. IN ADDITION TO USING GRANT FUNDS FOR THE ELIGIBLE COSTS DESCRIBED ABOVE, RECIPIENTS AND SUBRECIPIENTS IN CONTINUUMS OF CARE DESIGNATED AS HIGH PERFORMING COMMUNITIES MAY ALSO USE GRANT FUNDS TO PROVIDE HOUSING RELOCATION AND STABILIZATION SERVICES AND SHORT- AND/OR MEDIUM-TERM RENTAL ASSISTANCE TO INDIVIDUALS AND FAMILIES AT RISK OF HOMELESSNESS AS SET FORTH IN 24 CFR 576.103 AND 24 CFR 576.104, IF NECESSARY TO PREVENT THE INDIVIDUAL OR FAMILY FROM BECOMING HOMELESS. A SAFE HAVEN FOUNDATION'S (ASHF) MISSION IS TO HELP PEOPLE ASPIRE, TRANSFORM AND SUSTAIN THEIR LIVES AS THEY TRANSITION FROM HOMELESSNESS TO SELF-SUFFICIENCY WITH PRIDE AND PURPOSE. A SAFE HAVEN PROVIDES THE TOOLS FOR EACH INDIVIDUAL TO OVERCOME THE ROOT CAUSES OF HOMELESSNESS THROUGH A HOLISTIC AND SCALABLE MODEL. A SAFE HAVEN'S VISIBLE SOCIAL AND ECONOMIC IMPACT UNITES FAMILIES, STABILIZES NEIGHBORHOODS, AND CREATES VIBRANT, VIABLE COMMUNITIES. THE PROPOSED VISTA PROJECT ALIGNS WITH THE VISTA OPIOID PROGRAMMING FOCUS AREA. THE VISTA PROJECT WILL ENHANCE ASHF'S DATA ANALYTICS FOR THE PURPOSE OF IMPROVING TARGETED PROGRAM DELIVERY, WHILE SUPPORTING STAFF THROUGH TRAINING, CURRICULUM DEVELOPMENT, AND TARGETED OUTREACH. ASHF EXPECTS TO BENEFIT 5,000 BENEFICIARIES ANNUALLY. THE GOAL FOR EACH OF THE ROLES ARE TO ASSIST WITH THE SCALABILITY OF ASHF'S EXISTING SERVICES AND ENHANCE ORGANIZATIONAL CAPACITY BY CONTRIBUTING TO MORE DATA-DRIVEN DECISION-MAKING AND THE DEVELOPMENT OF A POPULATION HEALTH MANAGEMENT APPROACH TO OPIOID MANAGEMENT AND TREATMENT FOR CHICAGOANS AND ILLINOISANS. WE PROPOSE INCREASING THE NUMBER OF AMERICORPS VISTAS SERVING AT ASHF TO 14 MEMBERS IN ORDER TO ENHANCE ITS EVIDENCE-BASED PREVENTION AND TREATMENT INTERVENTIONS FOR THE PURPOSE OF REDUCING OPIOID USE AND ADDICTION AMONGST ASHF'S RESIDENTS. THE GOAL IS TO FURTHER ENHANCE ORGANIZATIONAL CAPACITY BY CREATING QUALITY IMPROVEMENT AND STANDARD OPERATING PROCEDURES ACROSS ASHF DIVISIONS. TARGET POPULATION-APPROXIMATELY 5000 STUDENTS/YOUNG ADULTS IN OUR COMMUNITY, MANY OF WHOM LIVE IN RELATIVE CAREGIVER SITUATIONS. "THIS PROJECT WILL FOCUS ON PROVIDING EDUCATION AND TRAINING TO THE YOUTH/PARENTS OF JOHNSON COUNTY TO RAISE THE AWARENESS OF THE DANGERS OF UNDERAGE ALCOHOL USE AND DRINKING AND DRIVING." ACCORDING TO DATA USA STATISTICAL RECORDS JOHNSON COUNTY IS A COMMUNITY OF 17,923 FULL TIME RESIDENTS. THE AVERAGE HOUSEHOLD INCOME IS $30,180, A 26.9% POVERTY RATE WITH 100% OF OUR STUDENTS RECEIVING FREE LUNCH. WE'RE AN ISOLATED COMMUNITY, APPROXIMATELY 1 HOUR FROM THE NEAREST INTERSTATE AND THE NEAREST FULL SERVICE HOSPITAL. APPROXIMATELY 15% OF OUR RESIDENTS ARE UNINSURED AND MANY MORE UNDERINSURED. WE HAVE NO TREATMENT FACILITIES FOR ALCOHOL OR SUD'S WITHIN AN HOURS DRIVE. WE HAVE ONE AA MEETING FOR THE ENTIRE COUNTY WHICH COVERS APPROXIMATELY 300 SQUARE MILES. WE HAVE 1 SCHOOL SYSTEM WITH 5 ELEMENTARY SCHOOLS AND ONE MIDDLE SCHOOL AND ONE HIGH SCHOOL. APPROXIMATELY ¼ OF OUR COUNTY POPULATION IS UNDER THE AGE OF 24. WE HAVE LIMITED INDUSTRY IN OUR COMMUNITY AND VERY LIMITED EMPLOYMENT RESOURCES. JOHNSON COUNTY SCHOOL SYSTEM AND NORTHEAST CORRECTIONAL CENTER ARE THE LARGEST EMPLOYERS WITHIN THE COUNTY. OUR YOUTH HAVE LIMITED ACCESS TO RECREATION SUCH AS MOVIES, BOWLING, SHOPPING AND OTHER ACTIVITIES AS NONE OF THESE ARE LOCATED WITHIN OUR COUNTY SO THE COMMON PRACTICE IS TO HAVE DRINKING PARTIES IN HOMES AND ISOLATED AREAS IN OUR COMMUNITY. WE RECENTLY PASSED LEGISLATION TO GO FROM THE LAST REMAINING "DRY" COUNTY IN THE UPPER EAST TENNESSEE AREA TO A "WET" COUNTY. GOAL 1: DECREASE YOUTH ALCOHOL USE IN THE COMMUNITY BY 5% OVER THE NEXT 4 YEARS BY IMPLEMENTING EVIDENCE-BASED PROGRAMS WITHIN THE SCHOOL DISTRICT THAT ADDRESS BEHAVIORS THAT MAY LEAD TO THE INITIATION OF USE. GOAL 2: STRENGTHEN COLLABORATION ACROSS COMMUNITY SECTORS TO CHANGE SOCIAL PERCEPTION OF RISK OF HARM AMONG PARENTS AND COMMUNITY. THIS WILL REACH ALL 17,923 RESIDENTS OVER THE 4 YEAR PERIOD. OBJECTIVE 1: BY THE END OF THE SCHOOL YEAR 2023, WE WILL HAVE UTILIZED THE BOTVIN LIFESKILLS TRAINING WITH APPROXIMATELY 200 STUDENTS AGES 12-15 TO DECREASE ALCOHOL CONSUMPTION BY 5% AND INCREASE AWARENESS OF HARM BY 5% AT ALL ELEMENTARY SCHOOLS AND WILL HAVE TRAINED 3 SCHOOL PERSONNEL WITH THIS SYSTEM. THIS EVIDENCE-BASED TRAINING PROVIDES SKILLS TO INCREASE GENERAL COMPETENCE AND REDUCE POTENTIAL MOTIVATIONS TO MISUSE/ABUSE SUBSTANCES. OBJECTIVE 2: BY MAY 2023, WE WILL HAVE INTEGRATED THE REALITY TOUR AND THE USE OF OUR ALCOHOL SIMULATOR PEDAL CAR AND OBSTACLE COURSE INTO OUR COMMUNITY . THIS TOUR WILL REACH 100 STUDENTS/PARENTS PER YEAR, 400-500 IN 4 YEARS. OBJECTIVE 3: OBJECTIVE 3: BY MAY 2023, WE WILL HAVE CONDUCTED SOBRIETY CHECKPOINTS, AND COMPLIANCE CHECKS ON A BIMONTHLY BASIS WORKING WITH LOCAL LAW ENFORCEMENT AND COMPLIANCE OFFICERS AT HOTSPOT LOCATIONS THROUGHOUT JOHNSON COUNTY INCLUDING HWY 67, HWY 421 AND IN TOWN AREA, HANDING OUT APPROXIMATELY 150 FLYERS PER EVENT ON UNDERAGE ALCOHOL CONSUMPTION AND IMPAIRED DRIVING. WE WILL WORK WITH LOCAL BEER BOARD TO SEE THAT FINES ARE ENFORCED AND ESTABLISHMENTS ARE REWARDED FOR BEING IN COMPLIANCE ON AN ANNUAL BASIS BY RECOGNITION IN LOCAL MEDIA AND ALSO ENSURE THOSE NOT IN COMPLIANCE ARE HELD ACCOUNTABLE BY PAYING FINES AS STATED BY BEER BOARD POLICY.
