| Department of Health and Human Services | A NEXT GENERATION DATA INFRASTRUCTURE TO UNDERSTAND DISPARITIES ACROSS THE LIFE COURSE - SUMMARY THIS INFRASTRUCTURE PROPOSAL AIMS TO FURTHER ADVANCE CAPABILITIES IN THE SOCIAL SCIENCES (BROADLY DEFINED) TO COLLECT DATA ON THE DAILY LIVES OF U.S. FAMILIES AND INDIVIDUALS. THESE DATA WILL BE MORE ACCURATE, MORE GRANULAR, AND MORE COMPREHENSIVE THAN IS CURRENTLY POSSIBLE IN TRADITIONAL SURVEY-BASED RESEARCH. THE CONTEXT FOR THIS IS THE UNDERSTANDING AMERICA STUDY (UAS), THE PROBABILITY-BASED INTERNET PANEL WE HAVE BEEN BUILDING AT USC SINCE 2014. THE INFRASTRUCTURE INCLUDES THE COMBINATION OF MANY DATA TYPES (INCLUDING SURVEY DATA, INFORMATION FROM WEARABLES, CONTEXTUAL AND ADMINISTRATIVE LINKAGES, ECOLOGICAL MOMENTARY ASSESSMENTS, SELF-RECORDED NARRATIVES, AND ELECTRONIC RECORDS OF FINANCIAL TRANSACTIONS), AS WELL AS AN OPEN COMMUNICATION WITH THE WIDER RESEARCH COMMUNITY BOTH IN DATA DISSEMINATION AND IN SOLICITING INPUT ON CONTENT AND METHODS. THE UAS CURRENTLY COMPRISES ABOUT 10,000 U.S. RESIDENTS (INCLUDING A 2,500 PERSON CALIFORNIA OVERSAMPLE), RECRUITED BY ADDRESS-BASED SAMPLING AND PROVIDED WITH INTERNET-ENABLED TABLETS IF NEEDED. SURVEYS ARE CONDUCTED IN ENGLISH AND SPANISH. WE PROPOSE TO EXPAND THE UAS NATIONAL SAMPLE TO 20,000 RESPONDENTS, WITH SUBSAMPLES OF BLACKS, ASIANS, AND HISPANICS OF AT LEAST 3,000 EACH. THE INFORMATION COLLECTED WILL FOCUS ON FACTORS EXPLAINING RACIAL AND SOCIO-ECONOMIC DISPARITIES OVER THE LIFE COURSE, INCLUDING RACIAL DISCRIMINATION, INEQUALITIES IN ACCESS TO EDUCATION AND HEALTHCARE, DIFFERENCES IN THE PHYSICAL AND SOCIAL ENVIRONMENT, AND, MORE GENERALLY, THE VARIOUS OPPORTUNITIES AND OBSTACLES ONE ENCOUNTERS OVER THE LIFE COURSE. THE PROPOSED PROJECT TEAM COVERS A BROAD SPECTRUM OF SUBSTANTIVE EXPERTISE ON DISPARITIES, RACIAL DISCRIMINATION, ENVIRONMENTAL HEALTH, CHILD DEVELOPMENT, HEALTH, AND COGNITION OVER THE LIFE COURSE, AS WELL AS AGING AND RETIREMENT, CONSUMPTION, WORK, AND INCOME. TO ENCOURAGE INPUT FROM THE RESEARCH COMMUNITY, WE ENVISION A PROGRAM OF ADDED SURVEY MODULES ON INEQUALITY-RELATED TOPICS PROPOSED BY OUTSIDE RESEARCHERS, AS WELL AS AN ANNUAL RESEARCH CONFERENCE. THE BASIC PREMISE UNDERLYING THE PROJECT IS THAT THE ONLY FEASIBLE APPROACH TO COMPREHENSIVELY INVESTIGATING THE IMPACT OF DIFFERENCES IN THE LIFE COURSE ON OBSERVED INEQUALITY IS TO ENACT A ROBUST MULTI-METHOD APPROACH THAT PROVIDES THE MEANS OF ACCOUNTING FOR AS MANY EXPLANATORY FACTORS AS POSSIBLE. WITHIN THAT CONTEXT, NATURALLY OCCURRING EXPERIMENTS CAN BE EXPLOITED TO HELP IDENTIFY CAUSAL PATHWAYS OF INTEREST (E.G., WHEN AIR QUALITY IN A NEIGHBORHOOD IMPROVES DUE TO A PLANT CLOSURE; A POLICY CHANGE THAT AFFECTS THE QUALITY OF SCHOOLS IN A CERTAIN AREA). THUS, OUR PROPOSED PROJECT AIMS TO CREATE AND MAKE AVAILABLE TO THE RESEARCH COMMUNITY A UNIQUELY DETAILED AND FOCUSED COLLECTION OF INFORMATION ON U.S. HOUSEHOLDS AND INDIVIDUALS. THE LANDSCAPE OF WHAT NEEDS TO BE PROTECTED AND FROM WHAT THREAT EVOLVES: NEW TECHNOLOGIES ARE RELEASED AND THE THREAT ACTORS IMPROVE THEIR OWN CAPABILITIES THROUGH EXPERIENCE AND CLOSE COLLABORATION. MEANWHILE, DEFENDERS WORK OFTEN IN ISOLATION, USING PRIVATE DATA AND FACILITIES, AND PRODUCING DEFENSES THAT ARE QUICKLY OUTPACED BY NEW THREATS. TO TRANSFORM CS&P RESEARCH INTO A HIGHLY INTEGRATED, COMMUNITY-WIDE EFFORT, CS&P RESEARCHERS NEED A COMMON, RICH, REPRESENTATIVE RESEARCH INFRASTRUCTURE, WHICH MEETS THE NEEDS ACROSS ALL MEMBERS OF THE COMMUNITY, AND FACILITATES REPRODUCIBLE SCIENCE. THE SPHERE RESEARCH INFRASTRUCTURE WILL OFFER A NOVEL MIX OF EXPERIMENTATION CAPABILITIES, UNIQUELY TAILORED TO THE NEEDS OF CS&P RESEARCHERS AND EDUCATORS. SPHERE?S NOVEL OFFERING OF DIVERSE, RICH HARDWARE INFRASTRUCTURE, CONFIGURABLE NETWORK SUBSTRATE AND SAFE NETWORK POLICIES WILL SUPPORT NOVEL CS&P RESEARCH IN EMERGING AREAS, SUCH AS IOT, CYBER-PHYSICAL SYSTEMS, PROGRAMMABLE NETWORKS, EDGE COMPUTING, INTERNET MEASUREMENT AND HUMAN-CENTRIC CS&P. SPHERE?S NOVEL USER PORTALS WILL DEMOCRATIZE ACCESS TO CS&P RESEARCH, AND WILL FACILITATE PRACTICAL CS&P EDUCATION OF BROAD STUDENT POPULATIONS. SPHERE?S NOVEL SUPPORT FOR REPRESENTATIVE EXPERIMENTATION AND REPRODUCIBILITY, TIGHT COLLABORATION WITH RESEARCHERS AND CLOSE ALLIANCES WITH ARTIFACT EVALUATION COMMITTEES, WILL ENABLE VERTICAL PROGRESS IN THE SCIENCE OF CS&P. THE SPHERE RESEARCH INFRASTRUCTURE WILL TRANSFORM CS&P RESEARCH, FROM PIECEMEAL AND OPPORTUNISTIC TO HIGHLY INTEGRATED, BY UNIFYING THE COMMUNITY?S EXPERIMENTATION EFFORTS ON A COMMON, RICH, HIGHLY USABLE INFRASTRUCTURE. INTEGRATED RESEARCH EFFORTS WILL INCREASE THE PACE OF INNOVATION AND IMPROVE THE SUCCESS AND SOPHISTICATION OF CS&P RESEARCH PRODUCTS. HISPANIC/LATINOS ARE THE LARGEST AND FASTING GROWING ETHNIC GROUP IN THE US, AND CANCER IS THE LEADING CAUSE OF DEATH AMONG H/L IN THE US. THEREFORE, WE NEED TO FULLY UNDERSTAND THE FULL COMPLEXITY OF THE MOLECULAR ETIOLOGY OF CANCER IN THIS ETHNIC GROUP. FOR INSTANCE, ALTHOUGH INCIDENCE RATES OF CRC ARE LOWER AMONG LATINOS AS COMPARED TO WHITES OR AFRICAN AMERICANS, HISPANICS WITH METASTATIC DISEASE HAVE SHORTER OVERALL SURVIVAL WHEN ADJUSTED FOR HEALTH CARE SETTING, DEMOGRAPHICS, DISEASE CHARACTERISTICS AND TREATMENT FACTORS. H/L ALSO TEND TO BE DIAGNOSED AT A YOUNGER AGE AND WITH HIGHER STAGE, AND WE HAVE PREVIOUSLY REPORTED THAT MEXICAN H/L IN CALIFORNIA HAVE THE GREATEST PROPORTION OF YOUNG (<50 YEARS OF AGE) DIAGNOSES COMPARED TO OTHER H/L SUBGROUPS. MOREOVER, MEXICAN H/L SHOWED HIGHER PREVALENCE OF RECTAL CANCER CASES COMPARED TO OTHER H/L AND NHW. ALTHOUGH SOCIO-ECONOMICS AND ACCESS TO CARE MIGHT INFLUENCE THESE DIFFERENCES, WE NEED TO TAKE A COMPLETE LOOK AT THE BIOLOGY OF DISEASE IN THIS ETHNIC GROUP TO DETERMINE ONCE AND FOR ALL IF THESE CLINICAL DIFFERENCES ARE RELATED TO DIFFERENCES IN MOLECULAR ETIOLOGY. THE CANCER GENOME ATLAS HAS PROVIDED A DEEP OVERVIEW OF THE MOLECULAR TAXONOMY OF CRC IN 594 CASES, HOWEVER, LESS THAN 1% OF THE CASES (N=5) WERE H/L. THEREFORE, IT IS IMPERATIVE FOR US TO TAKE MORE DETAILED ASSESSMENT OF THE MOLECULAR GENOMIC LANDSCAPE OF CRC IN H/L. ONE OF THE MAJOR ISSUES LIKELY LIMITING OUR ABILITY TO PERFORM THESE LARGE GENOMIC INITIATIVES IN MINORITY PATIENTS IS THAT PATIENT OR PARTICIPANT ENGAGEMENT PRACTICES MAY NOT BEEN INVESTIGATED TO IDENTIFY BEST PRACTICES FOR ACCRUING AND CONSENTING PATIENTS INTO CLINICAL TRANSLATIONAL BIOMEDICAL RESEARCH STUDIES. THIS CONCEPT OF PARTICIPANT ENGAGEMENT IS CRITICALLY IMPORTANT FOR BOTH THE PATIENTS AND THE TRANSLATIONAL CANCER RESEARCH COMMUNITY. OPTIMIZING AND IMPROVING OUR APPROACHES FOR DIRECTLY ENGAGING PATIENTS AT INITIAL CONTACT, THROUGHOUT THE COURSE OF A TRANSLATIONAL GENOMIC STUDY, AND DURING THE TIME OF RETURN OF RESULTS IS LIKELY TO LEAD TO STRONGER RELATIONSHIPS BETWEEN THE MEDICAL COMMUNITY AND PATIENTS, BUT COULD ALSO LEAD TO SIGNIFICANT IMPROVEMENT IN OUTCOMES FOR PATIENTS AND FOR THE CANCER CARE COMMUNITY AS A WHOLE. AS SUCH, WE PROPOSE THE CREATION OF THE USC CENTER FOR OPTIMIZATION OF PARTICIPANT ENGAGEMENT IN CANCER CHARACTERIZATION (COPECC) WITH A FOCUS ON OPTIMIZING THE ENGAGEMENT OF LATINOS IN CRC GENOMIC CHARACTERIZATION RESEARCH STUDIES. USC COPECC WOULD SERVE AS A MEMBER OF THE NCI U2C PARTICIPANT ENGAGEMENT AND CANCER GENOME SEQUENCING (PE-CGS) NETWORK. OUR INVESTIGATIVE TEAM INCLUDES EXPERTS IN ALL RELEVANT AREAS OF RESEARCH FOR GENOMIC CHARACTERIZATION, PARTICIPANT ENGAGEMENT, AND ENGAGEMENT OPTIMIZATION. WE HAVE AN ESTABLISHED PLATFORM FOR CONSENTING PATIENTS INTO CANCER GENOMICS STUDIES THAT WILL SERVE AS A STANDARD PROCESS. | Department of Health and Human Services | HORNET CENTER FOR AUTONOMIC NERVE RECORDING AND STIMULATION SYSTEMS (CARSS) - DESPITE A WIDE-RANGING INTEREST IN PERFORMING CLINICAL RESEARCH FOR BIOELECTRONIC MEDICINE APPLICATIONS, THERE ARE NO AVAILABLE OPEN-ARCHITECTURE AND OPEN-SOURCE IMPLANTABLE SYSTEMS FOR AUTONOMIC NERVE STIMULATION AND RECORDING. AS A RESULT, CLINICAL RESEARCHERS FACE SIGNIFICANT TECHNICAL, REGULATORY, AND FINANCIAL HURDLES IN GETTING ACCESS TO THE IMPLANTABLE NEUROMODULATION TECHNOLOGIES THAT ARE REQUIRED FOR PERFORMING THESE CLINICAL STUDIES. THERE ARE SEVERAL CLINICAL CLOSED-LOOP IMPLANTABLE NEUROMODULATION SYSTEMS PRESENTLY AVAILABLE AND THEY HAVE BEEN HELPFUL IN SUPPORTING CLINICAL RESEARCH. HOWEVER, IN THEIR CURRENT FORM, NONE ARE SUITABLE FOR THE BIOELECTRONIC MEDICINE APPLICATIONS, AS THEY LACK KEY FUNCTIONAL MODULES FOR ACCESSING THE AUTONOMIC NERVES; MOREOVER, MANY OF THEM USE CLOSED ARCHITECTURES (E.G., THE USE OF CUSTOM ASICS INSTEAD OF COMMERCIAL OVER-THE-SHELF COMPONENTS) AND PROPRIETARY SOFTWARE. THEREFORE, THE OVERALL OBJECTIVE OF THIS HORNET CENTER FOR AUTONOMIC NERVE RECORDING AND STIMULATION SYSTEMS (CARSS) IS TO DEVELOP AN OPEN-ARCHITECTURE AND OPEN-SOURCE IMPLANTABLE SYSTEM FOR AUTONOMIC NERVE STIMULATION AND RECORDING. WE PROPOSE TO DEVELOP THE CARSS IMPLANTABLE SYSTEM TAILORED FOR BIOELECTRONIC MEDICINE APPLICATIONS. THE CARSS SYSTEM INCLUDES AN EXTERNAL CHARGER AND CONTROLLER, IMPLANTABLE PULSE GENERATOR, AND AN ASSORTMENT OF INTEROPERABLE AND IMPLANTABLE LEADS FOR STIMULATION AND SENSING. AS SUCH, THIS SEAMLESSLY UNIFIED SYSTEM WILL ENABLE CLOSED-LOOP SENSING AND NEUROMODULATION, PROVIDING EXPERIMENTAL FLEXIBILITY AND CONTROL TO THE CLINICAL RESEARCH COMMUNITY. IN ORDER TO MAKE THE DEVELOPED SYSTEM AND ITS COMPONENTS AVAILABLE TO THE COMMUNITY, OUR CARSS CENTER WILL ENGAGE WITH THE USER COMMUNITY AND FACILITATE TECHNOLOGY DISSEMINATION BY PROVIDING SYSTEM TEMPLATES AND LIBRARIES (ALONG WITH SUPPORTING TECHNICAL DOCUMENTATION) AS WELL AS TRAINING. THE CENTRAL GOAL OF OUR DISSEMINATION APPROACH IS TO COLLABORATE WITH THE COMMUNITY TO REDUCE THE TECHNICAL, REGULATORY, AND FINANCIAL BARRIERS TO ENTRY INTO BIOELECTRONIC MEDICINE RESEARCH AND THEREBY ACCELERATE THE DEVELOPMENT AND TRANSLATION OF NOVEL BIOELECTRONIC MEDICAL THERAPIES. IN A SIMILAR VEIN, THIS DISCOVER EXPEDITION WILL PAVE THE WAY FOR SEMINAL INNOVATIONS IN INTEGRATED ELECTRONICS, SUSTAINABLE EXA-SCALE COMPUTING, AND ACCELERATION OF MACHINE LEARNING. DISCOVER WILL ENABLE COMPUTING PARADIGMS THAT CAN FACILITATE MODELING OF CLIMATE-CHANGE EFFECTS, DETECTION OF UNDERGROUND GEOLOGICAL RESOURCES, ENHANCEMENT OF PHARMACEUTICAL DRUG DESIGN FOR PERSONALIZED MEDICINES AND HEALTHCARE, AND DEVELOPMENT OF INNOVATIVE SMART MATERIALS AND INFRASTRUCTURE. WE WILL DO THIS BY (1) IDENTIFYING KEY EXPERTS AND STAKEHOLDERS ACROSS DISCIPLINES WITHIN EACH DOMAIN, (2) ORGANIZING THOSE EXPERTS AND STAKEHOLDERS THROUGH ACTIVITIES AIMED AT IDENTIFYING KEY EXPOSOME MEASURES AND CREATING CONSENSUS AROUND THEIR PRIORITIZATION AND DEFINITION AND, WHEN LACKING CONSENSUS, SUPPORTING RESEARCH TO FACILITATE CONSENSUS, (3) PROMOTING WITHIN AND CROSS-DOMAIN ENGAGEMENTS TO INSPIRE INNOVATION, UNDERSTANDING, THE CREATION OF COMMON MEASUREMENTS, AND COLLABORATIONS, AND (4) DISSEMINATING DATA, FINDINGS, AND GUIDANCE FROM THE GECC’S ACTIVITIES SO THAT THEY CAN BE USED BY THE BROADER COMMUNITY AND ESTABLISH A PUBLISHED RECORD OF OUR DECISION-MAKING AND INFORM FUTURE GENERATIONS OF RESEARCHERS. IN ADDITION TO LEADING THE DEVELOPMENT OF EXPOSURE MEASURES, THE GECC WILL ALSO PROMOTE AND SUPPORT OTHERS IN DEVELOPING AND HARMONIZING EXPOSOME DATA, INCLUDING NIA-SUPPORTED INFRASTRUCTURE PROJECTS, CENTER AND NETWORK PROGRAMS AND OTHER INTERESTED RESEARCHERS AND PROJECTS. THIS SUPPORT WILL COME THROUGH PUBLISHING DETAILED GUIDANCE DOCUMENTS, ORGANIZING CAPACITY-BUILDING WORKSHOPS, AND THE GECC’S PILOT PROGRAM. THE GECC WILL FURTHER ENCOURAGE INNOVATIONS AND PARTICIPATION FROM THE BROADER SCIENTIFIC COMMUNITY THROUGH PUBLICATIONS, RESEARCH BRIEFS, WEBINARS, HACKATHONS, EXHIBITS, AND SOCIAL MEDIA, AS WELL AS THE CENTER’S IN-PERSON AND VIRTUAL WORKSHOPS. PEOPLE WITH DS HAVE A LIFETIME RISK FOR DEMENTIA IN EXCESS OF 75% AND COMPRISE THE WORLD'S LARGEST POPULATION OF GENETICALLY-DETERMINED AD. JUST AS STUDYING DS HELPED IDENTIFY THE ROLE OF AMYLOID PRECURSOR PROTEIN MUTATIONS IN AD PATHOGENESIS, IT IS ALSO LIKELY TO INFORM US OF THE POTENTIAL BENEFIT OF MANIPULATING THE AMYLOID PATHWAY ON TREATMENT OUTCOMES IN AD. IT IS CRITICALLY IMPORTANT TO THE DS POPULATION AND TO THE AD THERAPEUTICS FIELD TO CONDUCT CLINICAL TRIALS, PARTICULARLY THOSE TARGETING AMYLOID ACCUMULATION, IN INDIVIDUALS WITH DS. WITH THIS APPLICATION, WE PROPOSE TO UTILIZE THE EXISTING DEPTH AND BREADTH OF EXPERTISE OF THE NIA-FUNDED ALZHEIMER'S CLINICAL TRIAL CONSORTIUM (ACTC) TO CONDUCT AD CLINICAL TRIALS IN ADULTS WITH DS ACROSS PERFORMANCE SITES WITH EXPERTISE IN DS INCLUDING THE ALZHEIMER'S BIOMARKER CONSORTIUM FOR DOWN SYNDROME (ABC-DS). AS PART OF AN NIH AWARD RECEIVED LAST YEAR, WE ESTABLISHED THE ACTC-DS NETWORK WHICH INCLUDES WORKING GROUPS COMPRISED OF LEADERS FROM ACTC, ABC-DS AND THE EUROPEAN HORIZON21 DS NETWORK TO DEVELOP THE COLLABORATIONS, INFRASTRUCTURE AND PLANS REQUIRED TO CONDUCT AD CLINICAL TRIALS IN DS. DURING THE PROPOSED R61 `TRIAL READINESS PHASE' OF THE PRESENT PROJECT, IN AIM 1, WE WILL ENROLL 120 ADULTS WITH DS (AGES 35-55) ACROSS 15 SITES INTO A TRIAL READY COHORT (TRC) TO COLLECT OUTCOME MEASURES AND BIOMARKERS HARMONIZED WITH THOSE BEING COLLECTED IN THE ONGOING ABC- DS STUDY (N = ~400). IN AIM 2, WE WILL DETERMINE THE RELATIONSHIPS BETWEEN COGNITIVE MEASURES AND AD BIOMARKERS TO IDENTIFY ENDPOINTS FOR CLINICAL TRIALS THAT BEST REFLECT DISEASE PROGRESSION. DURING THE R33 `CLINICAL TRIAL' PHASE, FOR AIM 3, WE PROPOSE TO IMPLEMENT A PHASE II RANDOMIZED, DOUBLE-BLIND, PLACEBO-CONTROLLED TRIAL TO EVALUATE THE SAFETY AND TOLERABILITY OF A PROMISING ANTI-AMYLOID THERAPEUTIC AMONG INDIVIDUALS THE TRC. IN AIM 4, WE WILL DETERMINE THE IMPACT OF THIS AGENT ON BIOMARKER EVIDENCE OF DISEASE MODIFICATION. FUNDAMENTALLY, THIS PROJECT WILL SERVE TO BRING AD THERAPIES TO THE DS POPULATION BY LEVERAGING THE INFRASTRUCTURE OF ACTC AND INCORPORATING THE EXPERIENCE AND EXPERTISE OF THE ABC-DS AND HORIZON21 NETWORKS. IN THIS INFRASTRUCTURE PROPOSAL, WE PROPOSE TO MAKE USE OF RECENT ADVANCES IN GENETICS BY GENOTYPING THE UNDERSTANDING AMERICA STUDY (UAS) AND CONSTRUCTING “POLYGENIC SCORES” (PGSS), INDEXES THAT AGGREGATE THE SMALL EFFECTS OF MILLIONS OF GENETIC VARIANTS FROM ACROSS THE GENOME, FOR USE IN SOCIAL-SCIENCE STUDIES OF FACTORS THAT INCREASE OR MITIGATE THE RISK OF ALZHEIMER’S DISEASE AND RELATED DEMENTIAS (ADRD). THE UAS, A PROBABILITY- BASED INTERNET PANEL HOUSED AT THE CENTER FOR ECONOMIC AND SOCIAL RESEARCH (CESR) AT THE UNIVERSITY OF SOUTHERN CALIFORNIA, LONGITUDINALLY TRACKS A SAMPLE OF APPROXIMATELY 10,000 ADULTS IN THE U.S. (GROWING TO AT LEAST 20,000 BY 2026). IT COMBINES SEVERAL SOURCES OF INFORMATION, INCLUDING FROM SURVEYS, WEARABLE DEVICES, ADMINISTRATIVE LINKAGES, AND CONTEXTUAL DATA, AND HAS SEVERAL UNIQUE FEATURES: IT PROVIDES THE OPPORTUNITY FOR ON- DEMAND DATA COLLECTION ON SHORT NOTICE; IT ALLOWS FOR THE COLLECTION OF DATA AT HIGHER FREQUENCIES AND FOR THE POSSIBILITY OF INITIATING NEW DATA COLLECTION IN RESPONSE TO MAJOR SOCIETAL EVENTS (SUCH AS THE COVID-19 PANDEMIC), OR TRIGGERED BY EVENTS IN THE LIVES OF RESPONDENTS (SUCH AS “BURST SURVEYS” FIELDED WHEN THERE IS AN IMPORTANT CHANGE IN THE LIFE OF A PANEL MEMBER); IT CAN BE USED TO TAKE ADVANTAGE OF NATURAL EXPERIMENTS; IT ALLOWS FOR FREQUENT COLLECTION (ONCE OR TWICE A MONTH) OF PARADATA (COMPUTER USER-BEHAVIOR FROM SURVEYS, E.G., ERRORS AND PROCESSING SPEED GLEANED FROM KEYSTROKES) WHICH IS PREDICTIVE OF COGNITIVE FUNCTIONING. OURS IS NOT A GENOME-WIDE ASSOCIATION STUDY (GWAS). INSTEAD, WE WILL USE GENETIC VARIANTS (SNPS) IDENTIFIED FROM EXISTING LARGE, REPLICATED GWASS, TO CREATE POLYGENIC SCORES (PGSS), AND EXPLOIT UNIQUE UAS CAPABILITIES, AFFORDED BY ITS INTERNET MODE OF OPERATION, TO BETTER UNDERSTAND ADRD RISK IN A NATIONALLY REPRESENTATIVE SAMPLE. MANY OF THESE FAULTS HAVE DIMENSIONS LARGE ENOUGH TO GENERATE HAZARDOUS EARTHQUAKES. THE SAN ANDREAS FAULT SYSTEM HAS AN EXCELLENT FIELD ACCESS THAT ALLOWS DETAILED STUDIES OF PROPERTIES AND DYNAMICS OF EARTHQUAKES AND FAULTING PROCESSES. THE STATEWIDE CALIFORNIA EARTHQUAKE CENTER (SCEC) IS A MULTI-DISCIPLINARY COLLABORATION THAT ENABLES THE DEVELOPMENT OF SYSTEM-LEVEL MODELS OF EARTHQUAKES AND THEIR HAZARDS. THE CENTER ADVANCES THE FRONTIERS OF EARTHQUAKE RESEARCH AND HAZARD ANALYSIS, AND RELATED STEM EDUCATION AND ENGAGEMENT, THROUGH FIVE MAJOR ACTIVITIES: (1) SYSTEM SCIENCE RESEARCH, (2) RESEARCH COMPUTING, (3) COMMUNITY ENGAGEMENT (4) WORKFORCE DEVELOPMENT, AND (5) COLLABORATION PLANNING AND PROGRAM EVALUATION. THE FOCUS OF THIS GEOHAZARD CENTER ON COUPLED EVOLUTION OF EARTHQUAKES AND FAULTS FACILITATES THE DEVELOPMENT OF QUANTITATIVE RELATIONS BETWEEN DIVERSE FEATURES THAT CAN BE USED FOR VALIDATION AND IMPROVED HAZARD ASSESSMENT. ADVANCED RESEARCH COMPUTING FOR BUILDING AND TESTING SOFTWARE AND MODELS IS USED TO VALIDATE RESULTS, IMPROVE UNCERTAINTY ESTIMATES, AND ASSESS THE EFFICACY OF RESEARCH ACTIVITIES TOWARD IMPROVING EARTHQUAKE SCIENCE AND IMPACTS ON SOCIETY. THIS PROJECT IS CO-FUNDED BY THE DIRECTORATE FOR GEOSCIENCES TO SUPPORT AI/ML ADVANCEMENT IN THE GEOSCIENCES. THE CENTER AIMS TO IDENTIFY EMERGENT PHENOMENA AND FUTURE BEHAVIOR OF EARTHQUAKES AND FAULTS USING THE SAN ANDREAS PLATE-BOUNDARY SYSTEM AS A NATURAL LABORATORY. THE RESEARCH ADDRESSES FIVE KEY QUESTIONS ON I) THE LOADING OF FAULTS, II) THE RESPONSE OF FAULT AND CRUSTAL MATERIALS TO LOADING, III) THE EVOLUTION OF THE ACTIVE FAULT NETWORK, IV) HOW THE EVOLVING PROPERTIES AFFECT ONGOING DEFORMATION AND GROUND MOTION, AND V) THE FORECASTING OF STRONG GROUND MOTION AND LARGE EARTHQUAKES. ADDRESSING THESE QUESTIONS COULD REVOLUTIONIZE EARTHQUAKE SCIENCE AND HAZARD ESTIMATION. THE CENTER?S SHARED RESEARCH COMPUTING ACTIVITIES AND COMMUNITY MODELS PROVIDE UNIQUE PATHWAYS FOR PHYSICS-BASED MODELING OF EARTHQUAKES AND THE GROUND MOTIONS THEY GENERATE. SCEC INTERACTS WITH FEDERAL AND STATE AGENCIES TO SUPPORT SEISMIC RISK REDUCTION AND PROMOTE COMMUNITY RESILIENCE. THE CENTER?S WORKING GROUPS, WORKSHOPS, FIELD ACTIVITIES, AND ANNUAL MEETINGS ENABLE A DIVERSE GROUP OF SCIENTISTS TO INTERACT OVER SUSTAINED PERIODS, BUILDING DEEP SCIENTIFIC COLLABORATIONS AND INTERPERSONAL NETWORKS. IN ADDITION, THE CENTER PROMOTES INTELLECTUAL EXCHANGE AND AMPLIFY THE SUPPORT FOR STUDENTS AND EARLY-CAREER SCIENTISTS, WHILE BROADENING THEIR PARTICIPATION IN STEM FIELDS. CONDUCTING THE RESEARCH, COMMUNITY ENGAGEMENT AND EDUCATION ACTIVITIES WITHIN THE ORGANIZATIONAL STRUCTURE OF SCEC FACILITATES PRODUCTIVE ACTIVITIES THAT OFTEN LEAD TO A LARGER IMPACT THAN THE SUM OF INDIVIDUAL PARTS. DESPITE HISTORICAL DECLINES IN SMOKING RATES, CIGARETTE SMOKING CONTINUES TO ACCOUNT FOR AN ESTIMATED 480,000 U.S. DEATHS PER YEAR, AND AVAILABLE PHARMACOTHERAPIES FOR SMOKING CESSATION ARE ONLY MODERATELY EFFECTIVE. OBESITY RATES CONTINUE TO RISE DRAMATICALLY: NEARLY ONE-HALF OF U.S. ADULTS WILL LIVE WITH OBESITY BY 2030. THE EMERGENCE OF HIGHLY EFFECTIVE INCRETIN-BASED THERAPIES FOR DIABETES AND OBESITY, INCLUDING LONG-ACTING GLUCAGON-LIKE-PEPTIDE 1 (GLP-1) RECEPTOR AGONISTS AND DUAL GLP-1/GIP (GASTRIC INHIBITORY PEPTIDE) RECEPTOR AGONISTS, IS LEADING TO RAPID ADVANCES IN THE CLINICAL MANAGEMENT OF OBESITY AND OTHER CARDIOMETABOLIC DISORDERS. IMPORTANTLY, PRECLINICAL AND EMERGING CLINICAL FINDINGS INDICATE THAT GLP-1 RECEPTOR AGONISTS REDUCE THE INTAKE OF ADDICTIVE DRUGS, INCLUDING NICOTINE. DATA FROM EARLY STUDIES WITH PEOPLE WHO SMOKE SUGGEST THAT GLP-1RA MAY INCREASE CIGARETTE ABSTINENCE AND PREVENT POST-CESSATION WEIGHT GAIN. CONSISTENT WITH THIS EVIDENCE, OUR PHASE II CLINICAL TRIAL DATA INDICATE THAT LOW-DOSE TREATMENT WITH A LONG-ACTING GLP-1 RECEPTOR AGONIST LEADS TO PROSPECTIVE REDUCTIONS IN CIGARETTE CRAVING AND CIGARETTES PER DAY IN NON-TREATMENT-SEEKING PEOPLE WHO SMOKE. LARGER RANDOMIZED TRIALS ARE NOW NEEDED TO DETERMINE THE EFFICACY OF INCRETIN-BASED THERAPIES FOR SMOKING CESSATION IN TREATMENT-SEEKING SAMPLES. TIRZEPATIDE, THE FIRST DUAL GLP-1/GIP RECEPTOR AGONIST, SHOWS SUPERIOR EFFICACY FOR WEIGHT LOSS COMPARED TO SEMAGLUTIDE. EVIDENCE FURTHER INDICATES THAT TIRZEPATIDE HAS PROTECTIVE EFFECTS ON CARDIOVASCULAR RISK OUTCOMES. THEREFORE, TIRZEPATIDE IS A PROMISING CANDIDATE THERAPY WITH POTENTIAL APPLICATIONS FOR CESSATION, REDUCTION OF CARDIOVASCULAR RISKS, AND PREVENTION OF POST-CESSATION WEIGHT GAIN. THIS MULTI-SITE, 24-WEEK CLINICAL TRIAL SEEKS TO EXPEDITE DATA ON THE EFFICACY OF TIRZEPATIDE FOR SMOKING CESSATION AND ASSOCIATED OUTCOMES (E.G., CIGARETTES PER DAY, POST-CESSATION WEIGHT GAIN, CARDIOVASCULAR RISK FACTORS) IN TREATMENT-SEEKING PEOPLE WHO SMOKE. | Department of Health and Human Services | VALIDATION OF IMAGING AND BLOOD-BASED SMALL VESSEL VCID BIOMARKERS IN MULTIETHNIC POPULATION - PROJECT SUMMARY/ABSTRACT THE MARKVCID CONSORTIUM WAS ESTABLISHED IN 2016 TO PURSUE THE INITIAL STAGES OF MULTI-SITE VALIDATION OF 7 IMAGING-BASED AND 4 FLUID-BASED CANDIDATE BIOMARKERS FOR SMALL VESSEL VASCULAR CONTRIBUTIONS TO COGNITIVE IMPAIRMENT AND DEMENTIA (VCID). AS ONE OF THE 7 PARTICIPATING SITES, OUR TEAM AT THE UNIVERSITY OF SOUTHERN CALIFORNIA (USC) HAS ANALYZED AND OPTIMIZED CANDIDATE VCID BIOMARKERS DURING THE UH2 PHASE (YEARS 1-2) AND PARTICIPATED IN THE CONSORTIUM-WIDE PROGRAM OF BIOMARKER SCALING-UP, MULTI-SITE PROTOCOL IMPLEMENTATION, AND INITIAL MULTI-SITE VALIDATION DURING THE UH3 PHASE (YEARS 3-5). USC HAS LED THE CONSORTIUM-WIDE DEVELOPMENT AND IMPLEMENTATION OF THE OPTICAL COHERENCE TOMOGRAPHY ANGIOGRAPHY (OCTA) BIOMARKER KIT WHICH IS BASED ON AN FDA-APPROVED DEVICE THAT ALLOWS IN VIVO AND COMPLETELY NONINVASIVE IMAGING OF RETINAL CAPILLARIES WITH A SPATIAL RESOLUTION OF ~10 MICRONS. WE HAVE ALSO PARTICIPATED IN THE VALIDATION OF THE OTHER 6 MRI BASED CANDIDATE BIOMARKERS WHILE CONTRIBUTING BLOOD SAMPLES FOR THE VALIDATION OF THE 3 PLASMA-BASED CANDIDATE BIOMARKERS. THE PRIMARY OBJECTIVE OF THE NEXT 5 YEARS IS TO CARRY OUT COMPREHENSIVE MULTI-SITE CLINICAL VALIDATION OF CANDIDATE VCID BIOMARKERS IN LONGITUDINAL STUDIES OF DIVERSE POPULATIONS THAT ARE TYPICAL IN CLINICAL SETTINGS IN THE US. CAPITALIZING ON THE HIGH RACIAL DIVERSITY IN LOS ANGELES, WE PLAN TO ENROLL A MULTIETHNIC LONGITUDINAL COHORT OF AT LEAST 200 SUBJECTS THAT ARE ENRICHED FOR SMALL VESSEL VCID, INCLUDING 100 LATINX SUBJECTS FROM THE LOS ANGELES LATINO EYE STUDY (LALES), 50 AFRICAN AMERICANS (AAS) ENROLLED IN THE AFRICAN AMERICAN EYE DISEASE STUDY (AFEDS), AND 50 CAUCASIAN PARTICIPANTS FROM THE CLINICAL CORE OF THE USC ADRC. WE HAVE PERFORMED PILOT STUDIES IN BOTH LATINX AND AA SUBJECTS (THE TWO LARGEST MINORITY GROUPS IN THE US) USING MARKVCID PROTOCOLS TO SHOW THE FEASIBILITY OF CONDUCTING SUCH STUDIES IN A MULTIETHNIC COHORT. WE HAVE ALSO BUILT A MULTIETHNIC TEAM OF INVESTIGATORS, TECHNICAL STAFF, RESEARCH COORDINATORS, AND COMMUNITY OUTREACH STAFF TO STREAMLINE ENROLLMENT, ACHIEVE THE RECRUITMENT MILESTONES, AND PERFORM LONGITUDINAL FOLLOW-UP STUDIES. OUR TEAM INCLUDES ESTABLISHED EXPERTISE, EQUIPMENT AND INFRASTRUCTURE FOR OCTA, MRI, FLUID BIOMARKER AND CLINICAL/COGNITIVE EVALUATION OF VCID, AS WELL AS A PROVEN TRACK RECORD IN THE SHARING OF DE-IDENTIFIED CLINICAL DATA THROUGH THE LABORATORY OF NEURO IMAGING (LONI) FOR THE SECOND PHASE OF MARKVCID PROJECT. | $5.4M | FY2021 | Sep 2021 – Jul 2026 |

A NEXT GENERATION DATA INFRASTRUCTURE TO UNDERSTAND DISPARITIES ACROSS THE LIFE COURSE - SUMMARY THIS INFRASTRUCTURE PROPOSAL AIMS TO FURTHER ADVANCE CAPABILITIES IN THE SOCIAL SCIENCES (BROADLY DEFINED) TO COLLECT DATA ON THE DAILY LIVES OF U.S. FAMILIES AND INDIVIDUALS. THESE DATA WILL BE MORE ACCURATE, MORE GRANULAR, AND MORE COMPREHENSIVE THAN IS CURRENTLY POSSIBLE IN TRADITIONAL SURVEY-BASED RESEARCH. THE CONTEXT FOR THIS IS THE UNDERSTANDING AMERICA STUDY (UAS), THE PROBABILITY-BASED INTERNET PANEL WE HAVE BEEN BUILDING AT USC SINCE 2014. THE INFRASTRUCTURE INCLUDES THE COMBINATION OF MANY DATA TYPES (INCLUDING SURVEY DATA, INFORMATION FROM WEARABLES, CONTEXTUAL AND ADMINISTRATIVE LINKAGES, ECOLOGICAL MOMENTARY ASSESSMENTS, SELF-RECORDED NARRATIVES, AND ELECTRONIC RECORDS OF FINANCIAL TRANSACTIONS), AS WELL AS AN OPEN COMMUNICATION WITH THE WIDER RESEARCH COMMUNITY BOTH IN DATA DISSEMINATION AND IN SOLICITING INPUT ON CONTENT AND METHODS. THE UAS CURRENTLY COMPRISES ABOUT 10,000 U.S. RESIDENTS (INCLUDING A 2,500 PERSON CALIFORNIA OVERSAMPLE), RECRUITED BY ADDRESS-BASED SAMPLING AND PROVIDED WITH INTERNET-ENABLED TABLETS IF NEEDED. SURVEYS ARE CONDUCTED IN ENGLISH AND SPANISH. WE PROPOSE TO EXPAND THE UAS NATIONAL SAMPLE TO 20,000 RESPONDENTS, WITH SUBSAMPLES OF BLACKS, ASIANS, AND HISPANICS OF AT LEAST 3,000 EACH. THE INFORMATION COLLECTED WILL FOCUS ON FACTORS EXPLAINING RACIAL AND SOCIO-ECONOMIC DISPARITIES OVER THE LIFE COURSE, INCLUDING RACIAL DISCRIMINATION, INEQUALITIES IN ACCESS TO EDUCATION AND HEALTHCARE, DIFFERENCES IN THE PHYSICAL AND SOCIAL ENVIRONMENT, AND, MORE GENERALLY, THE VARIOUS OPPORTUNITIES AND OBSTACLES ONE ENCOUNTERS OVER THE LIFE COURSE. THE PROPOSED PROJECT TEAM COVERS A BROAD SPECTRUM OF SUBSTANTIVE EXPERTISE ON DISPARITIES, RACIAL DISCRIMINATION, ENVIRONMENTAL HEALTH, CHILD DEVELOPMENT, HEALTH, AND COGNITION OVER THE LIFE COURSE, AS WELL AS AGING AND RETIREMENT, CONSUMPTION, WORK, AND INCOME. TO ENCOURAGE INPUT FROM THE RESEARCH COMMUNITY, WE ENVISION A PROGRAM OF ADDED SURVEY MODULES ON INEQUALITY-RELATED TOPICS PROPOSED BY OUTSIDE RESEARCHERS, AS WELL AS AN ANNUAL RESEARCH CONFERENCE. THE BASIC PREMISE UNDERLYING THE PROJECT IS THAT THE ONLY FEASIBLE APPROACH TO COMPREHENSIVELY INVESTIGATING THE IMPACT OF DIFFERENCES IN THE LIFE COURSE ON OBSERVED INEQUALITY IS TO ENACT A ROBUST MULTI-METHOD APPROACH THAT PROVIDES THE MEANS OF ACCOUNTING FOR AS MANY EXPLANATORY FACTORS AS POSSIBLE. WITHIN THAT CONTEXT, NATURALLY OCCURRING EXPERIMENTS CAN BE EXPLOITED TO HELP IDENTIFY CAUSAL PATHWAYS OF INTEREST (E.G., WHEN AIR QUALITY IN A NEIGHBORHOOD IMPROVES DUE TO A PLANT CLOSURE; A POLICY CHANGE THAT AFFECTS THE QUALITY OF SCHOOLS IN A CERTAIN AREA). THUS, OUR PROPOSED PROJECT AIMS TO CREATE AND MAKE AVAILABLE TO THE RESEARCH COMMUNITY A UNIQUELY DETAILED AND FOCUSED COLLECTION OF INFORMATION ON U.S. HOUSEHOLDS AND INDIVIDUALS. TO ACHIEVE THIS GOAL, WE AIM TO STRATEGICALLY INTEGRATE SELECTED EXTERNAL DATA SOURCES WITH UAS SURVEY DATA AND EXPAND THE ALREADY BROAD SET OF TOPICS COVERED BY UAS SURVEYS.

RECENT NEWSWORTHY ATTACKS TARGETED NATIONALLY IMPORTANT INFRASTRUCTURE, GOVERNMENT SYSTEMS, RESEARCHERS, AND RESEARCH FACILITIES. THE LANDSCAPE OF WHAT NEEDS TO BE PROTECTED AND FROM WHAT THREAT EVOLVES: NEW TECHNOLOGIES ARE RELEASED AND THE THREAT ACTORS IMPROVE THEIR OWN CAPABILITIES THROUGH EXPERIENCE AND CLOSE COLLABORATION. MEANWHILE, DEFENDERS WORK OFTEN IN ISOLATION, USING PRIVATE DATA AND FACILITIES, AND PRODUCING DEFENSES THAT ARE QUICKLY OUTPACED BY NEW THREATS. TO TRANSFORM CS&P RESEARCH INTO A HIGHLY INTEGRATED, COMMUNITY-WIDE EFFORT, CS&P RESEARCHERS NEED A COMMON, RICH, REPRESENTATIVE RESEARCH INFRASTRUCTURE, WHICH MEETS THE NEEDS ACROSS ALL MEMBERS OF THE COMMUNITY, AND FACILITATES REPRODUCIBLE SCIENCE. THE SPHERE RESEARCH INFRASTRUCTURE WILL OFFER A NOVEL MIX OF EXPERIMENTATION CAPABILITIES, UNIQUELY TAILORED TO THE NEEDS OF CS&P RESEARCHERS AND EDUCATORS. SPHERE?S NOVEL OFFERING OF DIVERSE, RICH HARDWARE INFRASTRUCTURE, CONFIGURABLE NETWORK SUBSTRATE AND SAFE NETWORK POLICIES WILL SUPPORT NOVEL CS&P RESEARCH IN EMERGING AREAS, SUCH AS IOT, CYBER-PHYSICAL SYSTEMS, PROGRAMMABLE NETWORKS, EDGE COMPUTING, INTERNET MEASUREMENT AND HUMAN-CENTRIC CS&P. SPHERE?S NOVEL USER PORTALS WILL DEMOCRATIZE ACCESS TO CS&P RESEARCH, AND WILL FACILITATE PRACTICAL CS&P EDUCATION OF BROAD STUDENT POPULATIONS. SPHERE?S NOVEL SUPPORT FOR REPRESENTATIVE EXPERIMENTATION AND REPRODUCIBILITY, TIGHT COLLABORATION WITH RESEARCHERS AND CLOSE ALLIANCES WITH ARTIFACT EVALUATION COMMITTEES, WILL ENABLE VERTICAL PROGRESS IN THE SCIENCE OF CS&P. THE SPHERE RESEARCH INFRASTRUCTURE WILL TRANSFORM CS&P RESEARCH, FROM PIECEMEAL AND OPPORTUNISTIC TO HIGHLY INTEGRATED, BY UNIFYING THE COMMUNITY?S EXPERIMENTATION EFFORTS ON A COMMON, RICH, HIGHLY USABLE INFRASTRUCTURE. INTEGRATED RESEARCH EFFORTS WILL INCREASE THE PACE OF INNOVATION AND IMPROVE THE SUCCESS AND SOPHISTICATION OF CS&P RESEARCH PRODUCTS. THUS, SPHERE WILL SIGNIFICANTLY ADVANCE SCIENTIFIC DISCOVERY AND THE NATION?S RESEARCH CAPABILITIES IN CS&P. USC PE-GCS: OPTIMIZING ENGAGEMENT OF HISPANIC COLORECTAL CANCER PATIENTS IN CANCER GENOMIC CHARACTERIZATION STUDIES - ABSTRACT COLORECTAL CANCER (CRC) IS THE SECOND LEADING CAUSE OF CANCER DEATH IN THE US. HISPANIC/LATINOS ARE THE LARGEST AND FASTING GROWING ETHNIC GROUP IN THE US, AND CANCER IS THE LEADING CAUSE OF DEATH AMONG H/L IN THE US. THEREFORE, WE NEED TO FULLY UNDERSTAND THE FULL COMPLEXITY OF THE MOLECULAR ETIOLOGY OF CANCER IN THIS ETHNIC GROUP. FOR INSTANCE, ALTHOUGH INCIDENCE RATES OF CRC ARE LOWER AMONG LATINOS AS COMPARED TO WHITES OR AFRICAN AMERICANS, HISPANICS WITH METASTATIC DISEASE HAVE SHORTER OVERALL SURVIVAL WHEN ADJUSTED FOR HEALTH CARE SETTING, DEMOGRAPHICS, DISEASE CHARACTERISTICS AND TREATMENT FACTORS. H/L ALSO TEND TO BE DIAGNOSED AT A YOUNGER AGE AND WITH HIGHER STAGE, AND WE HAVE PREVIOUSLY REPORTED THAT MEXICAN H/L IN CALIFORNIA HAVE THE GREATEST PROPORTION OF YOUNG (<50 YEARS OF AGE) DIAGNOSES COMPARED TO OTHER H/L SUBGROUPS. MOREOVER, MEXICAN H/L SHOWED HIGHER PREVALENCE OF RECTAL CANCER CASES COMPARED TO OTHER H/L AND NHW. ALTHOUGH SOCIO-ECONOMICS AND ACCESS TO CARE MIGHT INFLUENCE THESE DIFFERENCES, WE NEED TO TAKE A COMPLETE LOOK AT THE BIOLOGY OF DISEASE IN THIS ETHNIC GROUP TO DETERMINE ONCE AND FOR ALL IF THESE CLINICAL DIFFERENCES ARE RELATED TO DIFFERENCES IN MOLECULAR ETIOLOGY. THE CANCER GENOME ATLAS HAS PROVIDED A DEEP OVERVIEW OF THE MOLECULAR TAXONOMY OF CRC IN 594 CASES, HOWEVER, LESS THAN 1% OF THE CASES (N=5) WERE H/L. THEREFORE, IT IS IMPERATIVE FOR US TO TAKE MORE DETAILED ASSESSMENT OF THE MOLECULAR GENOMIC LANDSCAPE OF CRC IN H/L. ONE OF THE MAJOR ISSUES LIKELY LIMITING OUR ABILITY TO PERFORM THESE LARGE GENOMIC INITIATIVES IN MINORITY PATIENTS IS THAT PATIENT OR PARTICIPANT ENGAGEMENT PRACTICES MAY NOT BEEN INVESTIGATED TO IDENTIFY BEST PRACTICES FOR ACCRUING AND CONSENTING PATIENTS INTO CLINICAL TRANSLATIONAL BIOMEDICAL RESEARCH STUDIES. THIS CONCEPT OF PARTICIPANT ENGAGEMENT IS CRITICALLY IMPORTANT FOR BOTH THE PATIENTS AND THE TRANSLATIONAL CANCER RESEARCH COMMUNITY. OPTIMIZING AND IMPROVING OUR APPROACHES FOR DIRECTLY ENGAGING PATIENTS AT INITIAL CONTACT, THROUGHOUT THE COURSE OF A TRANSLATIONAL GENOMIC STUDY, AND DURING THE TIME OF RETURN OF RESULTS IS LIKELY TO LEAD TO STRONGER RELATIONSHIPS BETWEEN THE MEDICAL COMMUNITY AND PATIENTS, BUT COULD ALSO LEAD TO SIGNIFICANT IMPROVEMENT IN OUTCOMES FOR PATIENTS AND FOR THE CANCER CARE COMMUNITY AS A WHOLE. AS SUCH, WE PROPOSE THE CREATION OF THE USC CENTER FOR OPTIMIZATION OF PARTICIPANT ENGAGEMENT IN CANCER CHARACTERIZATION (COPECC) WITH A FOCUS ON OPTIMIZING THE ENGAGEMENT OF LATINOS IN CRC GENOMIC CHARACTERIZATION RESEARCH STUDIES. USC COPECC WOULD SERVE AS A MEMBER OF THE NCI U2C PARTICIPANT ENGAGEMENT AND CANCER GENOME SEQUENCING (PE-CGS) NETWORK. OUR INVESTIGATIVE TEAM INCLUDES EXPERTS IN ALL RELEVANT AREAS OF RESEARCH FOR GENOMIC CHARACTERIZATION, PARTICIPANT ENGAGEMENT, AND ENGAGEMENT OPTIMIZATION. WE HAVE AN ESTABLISHED PLATFORM FOR CONSENTING PATIENTS INTO CANCER GENOMICS STUDIES THAT WILL SERVE AS A STANDARD PROCESS. HORNET CENTER FOR AUTONOMIC NERVE RECORDING AND STIMULATION SYSTEMS (CARSS) - DESPITE A WIDE-RANGING INTEREST IN PERFORMING CLINICAL RESEARCH FOR BIOELECTRONIC MEDICINE APPLICATIONS, THERE ARE NO AVAILABLE OPEN-ARCHITECTURE AND OPEN-SOURCE IMPLANTABLE SYSTEMS FOR AUTONOMIC NERVE STIMULATION AND RECORDING. AS A RESULT, CLINICAL RESEARCHERS FACE SIGNIFICANT TECHNICAL, REGULATORY, AND FINANCIAL HURDLES IN GETTING ACCESS TO THE IMPLANTABLE NEUROMODULATION TECHNOLOGIES THAT ARE REQUIRED FOR PERFORMING THESE CLINICAL STUDIES. THERE ARE SEVERAL CLINICAL CLOSED-LOOP IMPLANTABLE NEUROMODULATION SYSTEMS PRESENTLY AVAILABLE AND THEY HAVE BEEN HELPFUL IN SUPPORTING CLINICAL RESEARCH. HOWEVER, IN THEIR CURRENT FORM, NONE ARE SUITABLE FOR THE BIOELECTRONIC MEDICINE APPLICATIONS, AS THEY LACK KEY FUNCTIONAL MODULES FOR ACCESSING THE AUTONOMIC NERVES; MOREOVER, MANY OF THEM USE CLOSED ARCHITECTURES (E.G., THE USE OF CUSTOM ASICS INSTEAD OF COMMERCIAL OVER-THE-SHELF COMPONENTS) AND PROPRIETARY SOFTWARE. THEREFORE, THE OVERALL OBJECTIVE OF THIS HORNET CENTER FOR AUTONOMIC NERVE RECORDING AND STIMULATION SYSTEMS (CARSS) IS TO DEVELOP AN OPEN-ARCHITECTURE AND OPEN-SOURCE IMPLANTABLE SYSTEM FOR AUTONOMIC NERVE STIMULATION AND RECORDING. WE PROPOSE TO DEVELOP THE CARSS IMPLANTABLE SYSTEM TAILORED FOR BIOELECTRONIC MEDICINE APPLICATIONS. THE CARSS SYSTEM INCLUDES AN EXTERNAL CHARGER AND CONTROLLER, IMPLANTABLE PULSE GENERATOR, AND AN ASSORTMENT OF INTEROPERABLE AND IMPLANTABLE LEADS FOR STIMULATION AND SENSING. AS SUCH, THIS SEAMLESSLY UNIFIED SYSTEM WILL ENABLE CLOSED-LOOP SENSING AND NEUROMODULATION, PROVIDING EXPERIMENTAL FLEXIBILITY AND CONTROL TO THE CLINICAL RESEARCH COMMUNITY. IN ORDER TO MAKE THE DEVELOPED SYSTEM AND ITS COMPONENTS AVAILABLE TO THE COMMUNITY, OUR CARSS CENTER WILL ENGAGE WITH THE USER COMMUNITY AND FACILITATE TECHNOLOGY DISSEMINATION BY PROVIDING SYSTEM TEMPLATES AND LIBRARIES (ALONG WITH SUPPORTING TECHNICAL DOCUMENTATION) AS WELL AS TRAINING. THE CENTRAL GOAL OF OUR DISSEMINATION APPROACH IS TO COLLABORATE WITH THE COMMUNITY TO REDUCE THE TECHNICAL, REGULATORY, AND FINANCIAL BARRIERS TO ENTRY INTO BIOELECTRONIC MEDICINE RESEARCH AND THEREBY ACCELERATE THE DEVELOPMENT AND TRANSLATION OF NOVEL BIOELECTRONIC MEDICAL THERAPIES. WE HAVE ASSEMBLED A COLLABORATIVE TEAM FROM INDUSTRY AND ACADEMIA TO ACHIEVE A CARSS SYSTEM SUITABLE FOR USE IN HUMAN CLINICAL RESEARCH.

WITH FUNDAMENTAL CMOS SCALING LIMITS CLOSE IN SIGHT, TIME IS NOW RIPE FOR AN EXPEDITION TO EXPLORE EMERGING DISRUPTIVE COMPUTING TECHNOLOGIES. THE DISCOVER EXPEDITION WILL EXPLORE NOVEL SUPERCONDUCTOR ELECTRONICS AS A VIABLE POST-CMOS COMPUTING TECHNOLOGY. SUPERCONDUCTOR ELECTRONICS CAN DELIVER ULTRA-HIGH PERFORMANCE AND ENERGY EFFICIENCY AT SCALE. THE POTENTIAL TRANSITION TO SUPERCONDUCTOR ELECTRONICS WILL BE REVOLUTIONARY, ANALOGOUS TO THE TRANSITION OF NMOS-ONLY LOGIC TO CMOS LOGIC IN THE 1970S. WHILE ENERGY EFFICIENCY WAS THE INITIAL DRIVER FOR THE TRANSITION TO CMOS, THE BENEFITS OF CMOS WENT FAR BEYOND ENERGY-EFFICIENCY GAINS AND INCLUDED ULTRA-SCALE INTEGRATION AND MORE RELIABLE CIRCUITS. IN A SIMILAR VEIN, THIS DISCOVER EXPEDITION WILL PAVE THE WAY FOR SEMINAL INNOVATIONS IN INTEGRATED ELECTRONICS, SUSTAINABLE EXA-SCALE COMPUTING, AND ACCELERATION OF MACHINE LEARNING. DISCOVER WILL ENABLE COMPUTING PARADIGMS THAT CAN FACILITATE MODELING OF CLIMATE-CHANGE EFFECTS, DETECTION OF UNDERGROUND GEOLOGICAL RESOURCES, ENHANCEMENT OF PHARMACEUTICAL DRUG DESIGN FOR PERSONALIZED MEDICINES AND HEALTHCARE, AND DEVELOPMENT OF INNOVATIVE SMART MATERIALS AND INFRASTRUCTURE. DISCOVER WILL HELP ENSURE THE PREEMINENCE OF THE US AS A TECHNOLOGICAL WORLD LEADER AND GUIDE FUTURE HARDWARE AND CHIP-MANUFACTURING INVESTMENTS. THE GENE ONTOLOGY (GO) IS BY FAR THE LARGEST KNOWLEDGEBASE OF HOW GENES FUNCTION, AND HAS BECOME A CRITICAL COMPONENT OF THE COMPUTATIONAL INFRASTRUCTURE ENABLING THE GENOMIC REVOLUTION. THE GO KNOWLEDGEBASE ENCODES A COMPUTATIONAL MODEL OF BIOLOGICAL SYSTEMS USING MODERN SEMANTIC TECHNOLOGIES, AND THIS IS THE KEY TO ITS BROAD ADOPTION AND APPLICATION. IT STORES VASTLY MORE KNOWLEDGE THAN ONE PERSON CAN KNOW, AND THEREFORE ENABLES COMPUTATIONAL ANALYSES THAT WOULD OTHERWISE BE IMPOSSIBLE. IT HAS BECOME INDISPENSABLE IN THE INTERPRETATION OF LARGE-SCALE MOLECULAR MEASUREMENTS IN BIOLOGICAL RESEARCH. LEVERAGING AND BUILDING UPON THE EXPERTISE GAINED FROM THE SUCCESSFUL DEVELOPMENT AND EXPANSION OF THE GATEWAY TO GLOBAL AGING DATA (THE GATEWAY) PLATFORM, WE PROPOSE TO ESTABLISH A RESEARCH COORDINATING CENTER, THE GATEWAY EXPOSOME COORDINATING CENTER (GECC) FOR AD/ADRD RESEARCH. GIVEN THE WIDE DIVERSITY IN EXPOSURES THAT OCCUR OVER THE LIFE COURSE, THERE IS A CRITICAL NEED TO FACILITATE DISCUSSIONS, ENGAGEMENTS, AND COLLABORATIONS BETWEEN RESEARCHERS AND STAKEHOLDERS ACROSS MANY DISCIPLINES TO CREATE A CENTRAL SOURCE OF MEASURES, GUIDANCE, AND TOOLS FOR THE SCIENTIFIC COMMUNITY WORKING ON AD/ADRD OUTCOMES. WE PROPOSE THE GECC TO BUILD CONSENSUS INCLUSIVELY AND TRANSPARENTLY AMONG EXPERTS AND KEY STAKEHOLDERS AROUND SIX KEY, INTERCONNECTED DOMAINS: CLIMATE, PHYSICAL ENVIRONMENT, SOCIAL ENVIRONMENT, POLICIES, COMMUNITY SERVICES, AND LIFE EXPERIENCES. THE GECC WILL SERVE AS A CENTRALIZED HUB FOR IDENTIFYING AND SETTING PRIORITIES, REACHING CONSENSUS, AND ESTABLISHING GUIDANCE ON ACCESSING, DEVELOPING, HARMONIZING, AND SHARING INNOVATIVE EXPOSOME MEASURES AND DATA FOR THE BROADER AD/ADRD RESEARCH AND STAKEHOLDER COMMUNITY. WE WILL DO THIS BY (1) IDENTIFYING KEY EXPERTS AND STAKEHOLDERS ACROSS DISCIPLINES WITHIN EACH DOMAIN, (2) ORGANIZING THOSE EXPERTS AND STAKEHOLDERS THROUGH ACTIVITIES AIMED AT IDENTIFYING KEY EXPOSOME MEASURES AND CREATING CONSENSUS AROUND THEIR PRIORITIZATION AND DEFINITION AND, WHEN LACKING CONSENSUS, SUPPORTING RESEARCH TO FACILITATE CONSENSUS, (3) PROMOTING WITHIN AND CROSS-DOMAIN ENGAGEMENTS TO INSPIRE INNOVATION, UNDERSTANDING, THE CREATION OF COMMON MEASUREMENTS, AND COLLABORATIONS, AND (4) DISSEMINATING DATA, FINDINGS, AND GUIDANCE FROM THE GECC’S ACTIVITIES SO THAT THEY CAN BE USED BY THE BROADER COMMUNITY AND ESTABLISH A PUBLISHED RECORD OF OUR DECISION-MAKING AND INFORM FUTURE GENERATIONS OF RESEARCHERS. IN ADDITION TO LEADING THE DEVELOPMENT OF EXPOSURE MEASURES, THE GECC WILL ALSO PROMOTE AND SUPPORT OTHERS IN DEVELOPING AND HARMONIZING EXPOSOME DATA, INCLUDING NIA-SUPPORTED INFRASTRUCTURE PROJECTS, CENTER AND NETWORK PROGRAMS AND OTHER INTERESTED RESEARCHERS AND PROJECTS. THE GECC WILL FURTHER ENCOURAGE INNOVATIONS AND PARTICIPATION FROM THE BROADER SCIENTIFIC COMMUNITY THROUGH PUBLICATIONS, RESEARCH BRIEFS, WEBINARS, HACKATHONS, EXHIBITS, AND SOCIAL MEDIA, AS WELL AS THE CENTER’S IN-PERSON AND VIRTUAL WORKSHOPS. COLLECTIVELY, THE ACTIVITIES OF THE GECC TO ESTABLISH CONSENSUS AROUND CORE AND INNOVATIVE EXPOSOME MEASURES AND PROMOTE THEIR HARMONIZATION AND DEVELOPMENT FOR BROAD USE WILL ENABLE AND ACCELERATE AD/ADRD RESEARCH ACROSS DISCIPLINES.

CLINICAL TRIALS TO PREVENT ALZHEIMER'S DISEASE IN DOWN SYNDROME - PROJECT SUMMARY/ABSTRACT THE DISCOVERY THAT INDIVIDUALS WITH TRISOMY 21, OR DOWN SYNDROME (DS) HAVE NEUROPATHOLOGICAL FEATURES IDENTICAL TO THOSE WITH SPORADIC ALZHEIMER'S DISEASE (AD) PLAYED A CRITICAL ROLE IN THE IDENTIFICATION OF THE AMYLOID PRECURSOR PROTEIN GENE ON CHROMOSOME 21 SUPPORTING THE AMYLOID CASCADE HYPOTHESIS. PEOPLE WITH DS HAVE A LIFETIME RISK FOR DEMENTIA IN EXCESS OF 75% AND COMPRISE THE WORLD'S LARGEST POPULATION OF GENETICALLY-DETERMINED AD. JUST AS STUDYING DS HELPED IDENTIFY THE ROLE OF AMYLOID PRECURSOR PROTEIN MUTATIONS IN AD PATHOGENESIS, IT IS ALSO LIKELY TO INFORM US OF THE POTENTIAL BENEFIT OF MANIPULATING THE AMYLOID PATHWAY ON TREATMENT OUTCOMES IN AD. IT IS CRITICALLY IMPORTANT TO THE DS POPULATION AND TO THE AD THERAPEUTICS FIELD TO CONDUCT CLINICAL TRIALS, PARTICULARLY THOSE TARGETING AMYLOID ACCUMULATION, IN INDIVIDUALS WITH DS. WITH THIS APPLICATION, WE PROPOSE TO UTILIZE THE EXISTING DEPTH AND BREADTH OF EXPERTISE OF THE NIA-FUNDED ALZHEIMER'S CLINICAL TRIAL CONSORTIUM (ACTC) TO CONDUCT AD CLINICAL TRIALS IN ADULTS WITH DS ACROSS PERFORMANCE SITES WITH EXPERTISE IN DS INCLUDING THE ALZHEIMER'S BIOMARKER CONSORTIUM FOR DOWN SYNDROME (ABC-DS). AS PART OF AN NIH AWARD RECEIVED LAST YEAR, WE ESTABLISHED THE ACTC-DS NETWORK WHICH INCLUDES WORKING GROUPS COMPRISED OF LEADERS FROM ACTC, ABC-DS AND THE EUROPEAN HORIZON21 DS NETWORK TO DEVELOP THE COLLABORATIONS, INFRASTRUCTURE AND PLANS REQUIRED TO CONDUCT AD CLINICAL TRIALS IN DS. DURING THE PROPOSED R61 `TRIAL READINESS PHASE' OF THE PRESENT PROJECT, IN AIM 1, WE WILL ENROLL 120 ADULTS WITH DS (AGES 35-55) ACROSS 15 SITES INTO A TRIAL READY COHORT (TRC) TO COLLECT OUTCOME MEASURES AND BIOMARKERS HARMONIZED WITH THOSE BEING COLLECTED IN THE ONGOING ABC- DS STUDY (N = ~400). IN AIM 2, WE WILL DETERMINE THE RELATIONSHIPS BETWEEN COGNITIVE MEASURES AND AD BIOMARKERS TO IDENTIFY ENDPOINTS FOR CLINICAL TRIALS THAT BEST REFLECT DISEASE PROGRESSION. DURING THE R33 `CLINICAL TRIAL' PHASE, FOR AIM 3, WE PROPOSE TO IMPLEMENT A PHASE II RANDOMIZED, DOUBLE-BLIND, PLACEBO-CONTROLLED TRIAL TO EVALUATE THE SAFETY AND TOLERABILITY OF A PROMISING ANTI-AMYLOID THERAPEUTIC AMONG INDIVIDUALS THE TRC. IN AIM 4, WE WILL DETERMINE THE IMPACT OF THIS AGENT ON BIOMARKER EVIDENCE OF DISEASE MODIFICATION. FUNDAMENTALLY, THIS PROJECT WILL SERVE TO BRING AD THERAPIES TO THE DS POPULATION BY LEVERAGING THE INFRASTRUCTURE OF ACTC AND INCORPORATING THE EXPERIENCE AND EXPERTISE OF THE ABC-DS AND HORIZON21 NETWORKS. BEYOND THE PROPOSED TRIAL, THIS WILL ESTABLISH THE MEANS AND METHODS TO CONDUCT FUTURE THERAPEUTIC TRIALS IN THIS POPULATION. THE POTENTIAL IMPACT OF THIS APPROACH ON IMPROVING THE LIVES OF ADULTS WITH DS AS WELL AS THE GENERAL POPULATION CANNOT BE OVERSTATED.

GENOTYPING THE UNDERSTANDING AMERICA STUDY TO GENERATE NOVEL OPPORTUNITIES FOR RESEARCH ON COGNITIVE FUNCTIONING AND DEMENTIA - PROJECT SUMMARY / ABSTRACT COGNITIVE IMPAIRMENT AND DEMENTIA ARE PREVALENT AND CAUSE SIGNIFICANT MORBIDITY AND SUBSTANTIAL FINANCIAL AND SOCIAL BURDEN. WITH THE RISING NUMBER OF CASES OF DEMENTIA IN THE U.S. AND WORLDWIDE, THERE IS AN URGENT NEED TO IDENTIFY OPPORTUNITIES FOR PREVENTING OR DELAYING ITS ONSET. IN THIS INFRASTRUCTURE PROPOSAL, WE PROPOSE TO MAKE USE OF RECENT ADVANCES IN GENETICS BY GENOTYPING THE UNDERSTANDING AMERICA STUDY (UAS) AND CONSTRUCTING “POLYGENIC SCORES” (PGSS), INDEXES THAT AGGREGATE THE SMALL EFFECTS OF MILLIONS OF GENETIC VARIANTS FROM ACROSS THE GENOME, FOR USE IN SOCIAL-SCIENCE STUDIES OF FACTORS THAT INCREASE OR MITIGATE THE RISK OF ALZHEIMER’S DISEASE AND RELATED DEMENTIAS (ADRD). THE UAS, A PROBABILITY- BASED INTERNET PANEL HOUSED AT THE CENTER FOR ECONOMIC AND SOCIAL RESEARCH (CESR) AT THE UNIVERSITY OF SOUTHERN CALIFORNIA, LONGITUDINALLY TRACKS A SAMPLE OF APPROXIMATELY 10,000 ADULTS IN THE U.S. (GROWING TO AT LEAST 20,000 BY 2026). IT COMBINES SEVERAL SOURCES OF INFORMATION, INCLUDING FROM SURVEYS, WEARABLE DEVICES, ADMINISTRATIVE LINKAGES, AND CONTEXTUAL DATA, AND HAS SEVERAL UNIQUE FEATURES: IT PROVIDES THE OPPORTUNITY FOR ON- DEMAND DATA COLLECTION ON SHORT NOTICE; IT ALLOWS FOR THE COLLECTION OF DATA AT HIGHER FREQUENCIES AND FOR THE POSSIBILITY OF INITIATING NEW DATA COLLECTION IN RESPONSE TO MAJOR SOCIETAL EVENTS (SUCH AS THE COVID-19 PANDEMIC), OR TRIGGERED BY EVENTS IN THE LIVES OF RESPONDENTS (SUCH AS “BURST SURVEYS” FIELDED WHEN THERE IS AN IMPORTANT CHANGE IN THE LIFE OF A PANEL MEMBER); IT CAN BE USED TO TAKE ADVANTAGE OF NATURAL EXPERIMENTS; IT ALLOWS FOR FREQUENT COLLECTION (ONCE OR TWICE A MONTH) OF PARADATA (COMPUTER USER-BEHAVIOR FROM SURVEYS, E.G., ERRORS AND PROCESSING SPEED GLEANED FROM KEYSTROKES) WHICH IS PREDICTIVE OF COGNITIVE FUNCTIONING. OURS IS NOT A GENOME-WIDE ASSOCIATION STUDY (GWAS). INSTEAD, WE WILL USE GENETIC VARIANTS (SNPS) IDENTIFIED FROM EXISTING LARGE, REPLICATED GWASS, TO CREATE POLYGENIC SCORES (PGSS), AND EXPLOIT UNIQUE UAS CAPABILITIES, AFFORDED BY ITS INTERNET MODE OF OPERATION, TO BETTER UNDERSTAND ADRD RISK IN A NATIONALLY REPRESENTATIVE SAMPLE. WE WILL USE PGSS, AS WELL AS APOE-E4 STATUS, TOGETHER WITH LONGITUDINAL HEALTH, COGNITIVE, BEHAVIORAL, AND ENVIRONMENTAL MEASURES, TO: (I) IDENTIFY POPULATIONS AT RISK OF COGNITIVE DECLINE, (II) COLLECT NEW DATA FOR CAUSAL INFERENCES OF THE EFFECTS OF ADRD RISK/PROTECTIVE FACTORS ON COGNITION BY GENETIC ADRD RISK, AND III) STUDY THE ROLE OF GENETICS IN THE RESILIENCE TO ADVERSE LIFE EVENTS AFFECTING COGNITIVE FUNCTIONING. BY MAKING PUBLICLY AVAILABLE A LARGE NUMBER OF GENETIC MEASURES FOR ADRD, COGNITIVE DECLINE, AND ASSOCIATED PROTECTIVE/RISK FACTORS (E.G., PHYSICAL ACTIVITY, CARDIOVASCULAR RISK [DIABETES, OBESITY, SMOKING AND HYPERTENSION], DIET, SLEEP, POLLUTION, AND EDUCATION, AMONG OTHERS), AND THROUGH OUR OWN RESEARCH, WE SEEK TO STIMULATE THE USE OF UNIQUE UAS CAPABILITIES IN ECONOMIC AND SOCIAL-SCIENCE RESEARCH OF ADRD, COGNITIVE IMPAIRMENT, AND COGNITIVE DECLINE.

CENTER OPERATIONS: THE COUPLED EVOLUTION OF EARTHQUAKES, FAULTS, AND GEOHAZARDS OF THE SAN ANDREAS FAULT SYSTEM -THE RAPIDLY DEFORMING PACIFIC-NORTH AMERICA PLATE BOUNDARY IN CALIFORNIA CONTAINS AN EVOLVING NETWORK OF HUNDREDS OF ACTIVE FAULTS ORGANIZED AROUND THE MAIN SAN ANDREAS FAULT. MANY OF THESE FAULTS HAVE DIMENSIONS LARGE ENOUGH TO GENERATE HAZARDOUS EARTHQUAKES. THE SAN ANDREAS FAULT SYSTEM HAS AN EXCELLENT FIELD ACCESS THAT ALLOWS DETAILED STUDIES OF PROPERTIES AND DYNAMICS OF EARTHQUAKES AND FAULTING PROCESSES. THE STATEWIDE CALIFORNIA EARTHQUAKE CENTER (SCEC) IS A MULTI-DISCIPLINARY COLLABORATION THAT ENABLES THE DEVELOPMENT OF SYSTEM-LEVEL MODELS OF EARTHQUAKES AND THEIR HAZARDS. THE CENTER ADVANCES THE FRONTIERS OF EARTHQUAKE RESEARCH AND HAZARD ANALYSIS, AND RELATED STEM EDUCATION AND ENGAGEMENT, THROUGH FIVE MAJOR ACTIVITIES: (1) SYSTEM SCIENCE RESEARCH, (2) RESEARCH COMPUTING, (3) COMMUNITY ENGAGEMENT (4) WORKFORCE DEVELOPMENT, AND (5) COLLABORATION PLANNING AND PROGRAM EVALUATION. THE FOCUS OF THIS GEOHAZARD CENTER ON COUPLED EVOLUTION OF EARTHQUAKES AND FAULTS FACILITATES THE DEVELOPMENT OF QUANTITATIVE RELATIONS BETWEEN DIVERSE FEATURES THAT CAN BE USED FOR VALIDATION AND IMPROVED HAZARD ASSESSMENT. ADVANCED RESEARCH COMPUTING FOR BUILDING AND TESTING SOFTWARE AND MODELS IS USED TO VALIDATE RESULTS, IMPROVE UNCERTAINTY ESTIMATES, AND ASSESS THE EFFICACY OF RESEARCH ACTIVITIES TOWARD IMPROVING EARTHQUAKE SCIENCE AND IMPACTS ON SOCIETY. THIS PROJECT IS CO-FUNDED BY THE DIRECTORATE FOR GEOSCIENCES TO SUPPORT AI/ML ADVANCEMENT IN THE GEOSCIENCES. THE CENTER AIMS TO IDENTIFY EMERGENT PHENOMENA AND FUTURE BEHAVIOR OF EARTHQUAKES AND FAULTS USING THE SAN ANDREAS PLATE-BOUNDARY SYSTEM AS A NATURAL LABORATORY. THE RESEARCH ADDRESSES FIVE KEY QUESTIONS ON I) THE LOADING OF FAULTS, II) THE RESPONSE OF FAULT AND CRUSTAL MATERIALS TO LOADING, III) THE EVOLUTION OF THE ACTIVE FAULT NETWORK, IV) HOW THE EVOLVING PROPERTIES AFFECT ONGOING DEFORMATION AND GROUND MOTION, AND V) THE FORECASTING OF STRONG GROUND MOTION AND LARGE EARTHQUAKES. ADDRESSING THESE QUESTIONS COULD REVOLUTIONIZE EARTHQUAKE SCIENCE AND HAZARD ESTIMATION. THE CENTER?S SHARED RESEARCH COMPUTING ACTIVITIES AND COMMUNITY MODELS PROVIDE UNIQUE PATHWAYS FOR PHYSICS-BASED MODELING OF EARTHQUAKES AND THE GROUND MOTIONS THEY GENERATE. SCEC INTERACTS WITH FEDERAL AND STATE AGENCIES TO SUPPORT SEISMIC RISK REDUCTION AND PROMOTE COMMUNITY RESILIENCE. THE CENTER?S WORKING GROUPS, WORKSHOPS, FIELD ACTIVITIES, AND ANNUAL MEETINGS ENABLE A DIVERSE GROUP OF SCIENTISTS TO INTERACT OVER SUSTAINED PERIODS, BUILDING DEEP SCIENTIFIC COLLABORATIONS AND INTERPERSONAL NETWORKS. IN ADDITION, THE CENTER PROMOTES INTELLECTUAL EXCHANGE AND AMPLIFY THE SUPPORT FOR STUDENTS AND EARLY-CAREER SCIENTISTS, WHILE BROADENING THEIR PARTICIPATION IN STEM FIELDS. NIDCD TEMPORAL BONE RESOURCE - PROJECT ABSTRACT THE ABILITY TO COLLECT AND SHARE DATA IS CRITICAL TO ADVANCES IN CLINICAL CARE AND IMPROVED HEALTHCARE. WHILE GENERAL COLLECTIONS OF DATA ASSOCIATED WITH GENES, PROTEINS, AND OTHER TYPES OF ASSAYS ARE AVAILABLE AND ESSENTIAL, SPECIALIZED DOMAINS REQUIRE MORE FOCUSED DATA REPOSITORIES TO ADEQUATELY REPRESENT THE UNIQUE ASPECTS OF THE RESEARCH DOMAIN. THIS IS ESPECIALLY TRUE OF DATA RELATED TO DEAFNESS AND COMMUNICATION DISORDERS. THE STUDY OF THE CAUSES AND TREATMENT OF THESE CONDITIONS OFTEN REQUIRES UNIQUE COMBINATIONS AND TYPES OF DATA. WITH THIS IN MIND, WE PROPOSE TO CREATE A UNIQUE SHARED DATA REPOSITORY, SPECIFICALLY TARGETED TOWARD THE RESEARCH PROJECTS FUNDED BY THE NATIONAL INSTITUTE FOR DEAFNESS AND COMMUNICATION DISORDERS. IN ORDER TO ACHIEVE RAPID DEPLOYMENT AND OPERATIONAL EFFICIENCY, WE WILL BUILD ON THE HIGHLY SUCCESSFUL FACEBASE DATA-SHARING INFRASTRUCTURE. SPECIFICALLY, WE PROPOSE TO PROVIDE THE NECESSARY INFRASTRUCTURE, SERVICES, AND PROCESSES FOR DATA MIGRATION, DATA COLLECTION, HANDLING OF PII/PHI, DISSEMINATION, AND DATA STEWARDSHIP FOR THE NIDCD TEMPORAL BONE DATABASE AND RELATED DATA RESOURCES. THE TEAM WILL CONSIST OF SOFTWARE ENGINEERING, DATA ENGINEERING, WEBSITE DESIGN AND DEVELOPMENT, AND TECHNICAL LEADERSHIP THROUGHOUT THE PERIOD OF PERFORMANCE. DURING THE FIRST YEAR, THE TEAM WILL PRIORITIZE THE MIGRATION OF DATA FROM EXISTING DATABASE(S), INCLUDING DATA THAT MAY INITIALLY CONTAIN PII/PHI, INTO THE FACEBASE INFRASTRUCTURE AND ESTABLISH A WEB- BASED PORTAL FOR ACCESS (SEARCH, UPLOADS, DOWNLOADS).

MULTI SITE RANDOMIZED TRIAL OF A GLP-1/GIP RECEPTOR AGONIST FOR SMOKING CESSATION - CIGARETTE SMOKING AND OBESITY ARE THE TWO LEADING PREVENTABLE CAUSES OF MORBIDITY AND MORTALITY IN THE UNITED STATES. DESPITE HISTORICAL DECLINES IN SMOKING RATES, CIGARETTE SMOKING CONTINUES TO ACCOUNT FOR AN ESTIMATED 480,000 U.S. DEATHS PER YEAR, AND AVAILABLE PHARMACOTHERAPIES FOR SMOKING CESSATION ARE ONLY MODERATELY EFFECTIVE. OBESITY RATES CONTINUE TO RISE DRAMATICALLY: NEARLY ONE-HALF OF U.S. ADULTS WILL LIVE WITH OBESITY BY 2030. THE EMERGENCE OF HIGHLY EFFECTIVE INCRETIN-BASED THERAPIES FOR DIABETES AND OBESITY, INCLUDING LONG-ACTING GLUCAGON-LIKE-PEPTIDE 1 (GLP-1) RECEPTOR AGONISTS AND DUAL GLP-1/GIP (GASTRIC INHIBITORY PEPTIDE) RECEPTOR AGONISTS, IS LEADING TO RAPID ADVANCES IN THE CLINICAL MANAGEMENT OF OBESITY AND OTHER CARDIOMETABOLIC DISORDERS. IMPORTANTLY, PRECLINICAL AND EMERGING CLINICAL FINDINGS INDICATE THAT GLP-1 RECEPTOR AGONISTS REDUCE THE INTAKE OF ADDICTIVE DRUGS, INCLUDING NICOTINE. DATA FROM EARLY STUDIES WITH PEOPLE WHO SMOKE SUGGEST THAT GLP-1RA MAY INCREASE CIGARETTE ABSTINENCE AND PREVENT POST-CESSATION WEIGHT GAIN. CONSISTENT WITH THIS EVIDENCE, OUR PHASE II CLINICAL TRIAL DATA INDICATE THAT LOW-DOSE TREATMENT WITH A LONG-ACTING GLP-1 RECEPTOR AGONIST LEADS TO PROSPECTIVE REDUCTIONS IN CIGARETTE CRAVING AND CIGARETTES PER DAY IN NON-TREATMENT-SEEKING PEOPLE WHO SMOKE. LARGER RANDOMIZED TRIALS ARE NOW NEEDED TO DETERMINE THE EFFICACY OF INCRETIN-BASED THERAPIES FOR SMOKING CESSATION IN TREATMENT-SEEKING SAMPLES. TIRZEPATIDE, THE FIRST DUAL GLP-1/GIP RECEPTOR AGONIST, SHOWS SUPERIOR EFFICACY FOR WEIGHT LOSS COMPARED TO SEMAGLUTIDE. EVIDENCE FURTHER INDICATES THAT TIRZEPATIDE HAS PROTECTIVE EFFECTS ON CARDIOVASCULAR RISK OUTCOMES. THEREFORE, TIRZEPATIDE IS A PROMISING CANDIDATE THERAPY WITH POTENTIAL APPLICATIONS FOR CESSATION, REDUCTION OF CARDIOVASCULAR RISKS, AND PREVENTION OF POST-CESSATION WEIGHT GAIN. THIS MULTI-SITE, 24-WEEK CLINICAL TRIAL SEEKS TO EXPEDITE DATA ON THE EFFICACY OF TIRZEPATIDE FOR SMOKING CESSATION AND ASSOCIATED OUTCOMES (E.G., CIGARETTES PER DAY, POST-CESSATION WEIGHT GAIN, CARDIOVASCULAR RISK FACTORS) IN TREATMENT-SEEKING PEOPLE WHO SMOKE. EXPEDITING PHASE II CLINICAL DATA ON THE EFFICACY OF TIRZEPATIDE IN TREATMENT-SEEKING PEOPLE WHO SMOKE WILL HELP TO INFORM DOSE SELECTION AND POPULATIONS OF FOCUS FOR PIVOTAL PHASE III CLINICAL TRIALS.

VALIDATION OF IMAGING AND BLOOD-BASED SMALL VESSEL VCID BIOMARKERS IN MULTIETHNIC POPULATION - PROJECT SUMMARY/ABSTRACT THE MARKVCID CONSORTIUM WAS ESTABLISHED IN 2016 TO PURSUE THE INITIAL STAGES OF MULTI-SITE VALIDATION OF 7 IMAGING-BASED AND 4 FLUID-BASED CANDIDATE BIOMARKERS FOR SMALL VESSEL VASCULAR CONTRIBUTIONS TO COGNITIVE IMPAIRMENT AND DEMENTIA (VCID). AS ONE OF THE 7 PARTICIPATING SITES, OUR TEAM AT THE UNIVERSITY OF SOUTHERN CALIFORNIA (USC) HAS ANALYZED AND OPTIMIZED CANDIDATE VCID BIOMARKERS DURING THE UH2 PHASE (YEARS 1-2) AND PARTICIPATED IN THE CONSORTIUM-WIDE PROGRAM OF BIOMARKER SCALING-UP, MULTI-SITE PROTOCOL IMPLEMENTATION, AND INITIAL MULTI-SITE VALIDATION DURING THE UH3 PHASE (YEARS 3-5). USC HAS LED THE CONSORTIUM-WIDE DEVELOPMENT AND IMPLEMENTATION OF THE OPTICAL COHERENCE TOMOGRAPHY ANGIOGRAPHY (OCTA) BIOMARKER KIT WHICH IS BASED ON AN FDA-APPROVED DEVICE THAT ALLOWS IN VIVO AND COMPLETELY NONINVASIVE IMAGING OF RETINAL CAPILLARIES WITH A SPATIAL RESOLUTION OF ~10 MICRONS. WE HAVE ALSO PARTICIPATED IN THE VALIDATION OF THE OTHER 6 MRI BASED CANDIDATE BIOMARKERS WHILE CONTRIBUTING BLOOD SAMPLES FOR THE VALIDATION OF THE 3 PLASMA-BASED CANDIDATE BIOMARKERS. THE PRIMARY OBJECTIVE OF THE NEXT 5 YEARS IS TO CARRY OUT COMPREHENSIVE MULTI-SITE CLINICAL VALIDATION OF CANDIDATE VCID BIOMARKERS IN LONGITUDINAL STUDIES OF DIVERSE POPULATIONS THAT ARE TYPICAL IN CLINICAL SETTINGS IN THE US. CAPITALIZING ON THE HIGH RACIAL DIVERSITY IN LOS ANGELES, WE PLAN TO ENROLL A MULTIETHNIC LONGITUDINAL COHORT OF AT LEAST 200 SUBJECTS THAT ARE ENRICHED FOR SMALL VESSEL VCID, INCLUDING 100 LATINX SUBJECTS FROM THE LOS ANGELES LATINO EYE STUDY (LALES), 50 AFRICAN AMERICANS (AAS) ENROLLED IN THE AFRICAN AMERICAN EYE DISEASE STUDY (AFEDS), AND 50 CAUCASIAN PARTICIPANTS FROM THE CLINICAL CORE OF THE USC ADRC. WE HAVE PERFORMED PILOT STUDIES IN BOTH LATINX AND AA SUBJECTS (THE TWO LARGEST MINORITY GROUPS IN THE US) USING MARKVCID PROTOCOLS TO SHOW THE FEASIBILITY OF CONDUCTING SUCH STUDIES IN A MULTIETHNIC COHORT. WE HAVE ALSO BUILT A MULTIETHNIC TEAM OF INVESTIGATORS, TECHNICAL STAFF, RESEARCH COORDINATORS, AND COMMUNITY OUTREACH STAFF TO STREAMLINE ENROLLMENT, ACHIEVE THE RECRUITMENT MILESTONES, AND PERFORM LONGITUDINAL FOLLOW-UP STUDIES. OUR TEAM INCLUDES ESTABLISHED EXPERTISE, EQUIPMENT AND INFRASTRUCTURE FOR OCTA, MRI, FLUID BIOMARKER AND CLINICAL/COGNITIVE EVALUATION OF VCID, AS WELL AS A PROVEN TRACK RECORD IN THE SHARING OF DE-IDENTIFIED CLINICAL DATA THROUGH THE LABORATORY OF NEURO IMAGING (LONI) FOR THE SECOND PHASE OF MARKVCID PROJECT.
